右美托咪啶复合氯胺酮用于小儿骶管麻醉的临床观察

摘要 目的探讨右美托咪啶复合氯胺酮用于小儿骶管阻滞麻醉的安全性及有效性。方法选择我院90例ASA分级Ⅰ级在骶管阻滞下行腹股沟疝手术的小儿患者,随机分为三组:氯胺酮组(K组)、丙泊酚复合氯胺酮组(PK组)和右美托咪啶复合氯胺酮组(DK组),每组30例。入室后三组患儿均静脉给予氯胺酮1mg/kg,待患儿安静入睡后行骶管阻滞。PK组缓慢静脉注射丙泊酚2mg/kg,随后以4mg/kg^(-1)·h^(-1)泵注维持;DK组给予右美托咪定0.5ug/kg负荷剂量,15min泵完,随后以0.25ug/kg^(-1)·h^(-1)泵注维持;K组泵入等容量生理盐水。若术中患儿出现体动反应时则分次静脉追加氯胺酮0.5mg/kg。观察三组患儿入室时(T0)、手术开始前(T1)、手术切皮时(T2)及手术结束时(T3)的血压、心率及脉搏血氧饱和度,记录术中患儿体动次数、氯胺酮用量、呼吸抑制发生情况、术后苏醒时间及苏醒期躁动发生情况。结果与K组比较,PK组和DK组血压和心率均降低,术中体动次数及氯胺酮用量减少,术后苏醒时间及苏醒期躁动发生率降低,呼吸抑制发生率PK组升高,DK组降低(P<0.05);与PK组比较,DK组围术期血压、心率更平稳,术中体动次数及氯胺酮用量减少,呼吸抑制及苏醒期躁动发生率降低(P<0.05),术后苏醒时间比较差异无统计学意义(P>0.05)。结论右美托咪定复合氯胺酮在小儿骶管阻滞中的应用具有良好的血流动力学作用及较好的术后恢复,是安全有效的麻醉方式。
