对精神分裂症患者行阿立哌唑替换氯丙嗪治疗的临床疗效分析

Analysis of the clinical efficacy of aripiprazole in place of chlorpromazine in patients with schizophrenia

目的探讨阿立哌唑替换传统精神病药物治疗精神分裂症(以阴性症状为主)患者的临床效果。方法收集2017年6月至2019年7月在我院治疗的所有精神分裂症患者中选取满足研究条件的108例患者,按照随机数字表法将上述患者分为两组,观察组(n=54)采用阿立哌唑进行治疗,对照组(n=54)采用氯丙嗪进行治疗,比较两组患者临床疗效、治疗前后阴性症状评分、总体印象评分、治疗后社会功能评分及不良反应发生情况。结果观察组临床治疗总有效率高于对照组,不良反应总发生率低于对照组,差异有统计学意义(P<0.05);治疗4、8、12周时,观察组的阴性症状量表(SANS)评分均明显低于对照组,临床总体印象(CGI-SI)量表评分均明显低于对照组,差异均有统计学意义(P<0.05);观察组患者治疗后住院精神患者社会功能评定量表(SSPI)(主动性、日常生活能力、社会活动技能)评分均明显高于对照组,差异均有统计学意义(P<0.05)。结论与传统精神病药物相比,采用阿立哌唑治疗以阴性症状为主的精神分裂症患者,临床效果显著,能够提高临床总有效率,改善阴性症状,降低临床总体印象,提高患者社会功能且不良反应少,临床用药安全性高,可在临床实践中进行广泛推广。

Objective To explore the clinical effect of aripiprazole in place of chlorpromazine in patients with schizophrenia(mainly negative symptoms).Methods A total of 108 patients with schizophrenia who met the research conditions in our hospital from June 2017 to July 2019 were selected.The above patients were divided into two groups according to the random number table method.The observation group(n=54)was treated with aripiprazole.The control group(n=54)was treated with chlorpromazine.Clinical efficacy,negative symptom score before and after treatment,overall impression score,social function score after treatment and incidence of adverse reactions were compared between two groups.Results The total effective rate of clinical treatment in the observation group was higher than that of the control group,and the total incidence of adverse reactions in the observation group was lower than that of the control group.The difference was statistically significant(P<0.05).At four weeks,eight weeks,and 12 weeks of treatment,the scores of the negative symptom scale(SANS)of the observation group were significantly lower than those of the control group.The scores of the clinical general impression(CGI-SI)scale in the observation group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).After treatment,the scores of scale of social-skills for psychiatric inpatients(SSPI)(proactive,daily living ability,social activity skills)of the observation group were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Compared with traditional psychiatric drugs,the use of aripiprazole in the treatment of schizophrenia with negative symptoms has a significant clinical effect,which can increase the total clinical effective rate,improve negative symptoms,reduce the overall clinical impression,and improve the social function of patients.It has few adverse reactions and high safety of the clinical medication,and can be widely promoted in clinical practice.
