正常精浆替换精浆液化不良患者异常精浆对精子前向运动影响研究

Study on the effect of abnormal seminal plasma on the forward movement of sperm in patients with poor seminal plasma liquefaction

目的:观察精液液化不良患者异常精浆替换为正常精浆后精子前向运动变化,探讨此疾病精子活动力下降原因。方法:将28例精液液化不良组(液化不良组)和28例正常精液组(液化正常组)两组每例精液样本都进行同样密度梯度离心和机械离心得到相应的离心后精子和不含精子的精浆。液化不良组异常精浆和液化正常组精浆互换,精子精浆交叉混匀,观察两组精子前向运动精子百分率(percentage of forward motile sperm,PR)变化。结果:精浆替换前液化不良组和液化正常组精子PR分别为(21.57±3.63)%和(45.26±9.64)%,差异有统计学意义(P<0.05);精浆替换后精子PR分别为(40.83±5.62)%和(30.89±4.57)%,差异无统计学意义(P>0.05)。结论:精浆异常是导致精液液化不良患者精子活动力下降重要因素,治疗因精液液化不良导致男性不育主要是针对精浆异常。

Objective:To observe the changes in the forward movement of sperm in patients with poor semen liquefaction after replacing the abnormal seminal plasma with normal seminal plasma,and to explore the causes of the decreased sperm motility in this disease.Methods:A total of 28 cases of poor semen liquefaction group(poor liquefaction group)and 28 cases of normal semen group(normal liquefaction group)were included in this study.The semen samples of each case in the two groups were subjected to the same density gradient centrifugation and mechanical centrifugation steps to obtain the corresponding centrifuged sperm and sperm-free seminal plasma.The abnormal seminal plasma of the non-liquefaction group and the seminal plasma of the normal liquefaction group were interchanged,and the seminal plasma was cross-mixed,and the changes in the percentage of spermatozoa(PR)of the two groups were observed.Results:The sperm PR of the poor liquefaction group before seminal plasma replacement and the normal liquefaction group were(21.57±3.63)%and(45.26±9.64)%,respectively,the difference was statistically significant(P<0.05);the sperm PR after seminal plasma replacement was(40.83±5.62)%and(30.89±4.57)%,the difference was not statistically significant(P>0.05).Conclusion:Abnormal seminal plasma is an important factor leading to the decline of sperm motility in patients with poor semen liquefaction.The treatment of male infertility due to poor semen liquefaction is mainly for seminal abnormalities.
