内窥镜下与传统切开获取大隐静脉患者临床预后及静脉桥血管术后下肢并发症的比较研究探寻

Clinical Outcomes of Patients with Great Saphenous Vein Under Endoscopy and Conventional Incision and Lower Extremity Complications After Venous Bridger Comparative Research Exploration

目的:对比内窥镜下与传统切开获取大隐静脉患者临床预后情况,评价静脉桥血管术后下肢并发症情况。方法:研究时间为2017年1月~2017年2月,研究对象为本院在该时间内收治的行单纯冠状动脉旁路移植术患者100例,其中内窥镜下获取大隐静脉患者50例纳入观察组,将传统切开获取大隐静脉的50例患者纳入对照组,分析两组患者手术指标、早期预后以及静脉桥血管术后下肢并发症发生情况等。结果:所有患者围手术期均无心梗发生,且均成功出院;两组患者ICU停留时间、住院时间差异无统计学意义(P>0.05);并发症发生率方面,对照组二次缝合愈合率、下肢疼痛+麻木率高于观察组,存在统计学意义(P<0.05),两组患者下肢血肿、感染方面无统计学意义(P>0.05),随访期间患者均无死亡。结论:内窥镜下与传统切开获取大隐静脉患者的短期预后相当,传统切开并发症以二次缝合愈合、下肢疼痛+麻木为主,内窥镜下以血肿为主,各有特点。

Objective:To compare the clinical prognosis of endoscopic and traditional incision for the great saphenous vein,and to evaluate the complications of lower limbs after vein bridger.Methods:The study time:January 2017 to February 2017,the object of study for the hospital treated within the time line of pure 100 patients with coronary artery bypass grafting,the endoscope for 50 patients with great saphenous vein into the observation group,the traditional incision for great saphenous vein of 50 cases in control group,operation index,analysis of two groups of patients early prognosis and vascular postoperative lower limb venous bridge complications occurred.Results:All patients had no myocardial infarction during the perioperative period,and they were all discharged successfully;there was no significant difference in ICU stay and hospital stay between the two groups(P>0.05);In terms of the incidence of complications,the rate of secondary suture healing and lower limb pain+numbness in the control group were higher than those in the observation group,which was statistically significant(P<0.05);The two groups of patients had no statistical significance in terms of lower limb hematoma and infection(P>0.05),and there was no death during the follow-up period.Conclusion:The short-term prognosis of patients with great saphenous vein under endoscopy is similar to that of patients with traditional incision.The complications of traditional incision are mainly secondary suture healing,lower limb pain+numbness,while hematoma is the main complication under endoscopy,each with its own characteristics.
