摘要
目的:应用超声心动图评价左心室血栓形成的影响因素。方法:选取40例左心室增大并血栓形成患者为血栓组(A组),另选取40例左心室增大无血栓形成患者组(B组),记录患者基本资料。对两组患者进行经胸超声心动图检查,测量两组患者左室舒张末内径(LVED)、左室收缩末内径(LVSD)、左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)、左室射血分数(LVEF)、左心室内血流速度(LV-v)、二尖瓣流速(MV)、主动脉瓣流速(AV)、组织多普勒二尖瓣环组织运动速度(TDI),进行统计学分析。结果:A组LVEF、LVA-v、MV、AV,TDI小于B组,LVED、LVSD、LVEDV、LVESV大于B组(P<0.05)。多因素Logistics回归分析显示LVED>57.5mm,LVEF<40.5%,二尖瓣流速E<45cm/s,TDI E′<4cm/s与左心室血栓形成相关。结论:左心室血栓形成与左室结构及功能改变相关,且有一定预测价值。
Objective:Echocardiography was used to evaluate the influencing factors of left ventricular thrombosis.Methods:Forty patients with enlarged left ventricle and thrombosis were selected as thrombosis group(group A),and 40 patients with enlarged left ventricle and no thrombosis group(group B),and the basic data of the patients were recorded.The left ventricular end diastolic diameter(LVED),left ventricular end systolic diameter(LVSD),left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV),left ventricular ejection fraction(LVEF)and left ventricular blood flow velocity(LV-v)were measured mitral valve velocity(MV),aortic valve velocity(AV),tissue Doppler mitral annular velocity(TDI)were analyzed statistically.Results:Group A,LVEF,LVA-v,MV,AV,TDI were less than group B,LVED,LVSD,LVEDV,LVESV were greater than group B(P<0.05).Multivariate Logistics regression analysis showed that LVED>57.5mm,LVEF<40.5%,mitral velocity E<45cm/s,TDI E′<4cm/s were associated with left ventricular thrombosis.Conclusion:Left ventricular thrombosis is correlated with the structural and functional changes of the left ventricle and has certain predictive value.
