摘要
目的:髂胫束综合征在跑步爱好者中发生率极高,本研究的目的是探究采用髋部强化训练对患有髂胫束综合征的女性跑步爱好者的疗效,为髂胫束综合征的运动疗法提供参考。方法:选取60名患有髂胫束综合征的女性跑步者,随机分为髂胫束拉伸组(A,n=20),常规运动组(B、n=20),髋关节强化运动组(C、n=20)。A组采用髂胫束拉伸训练,拉伸3次/天,每次10min,每天30min,每周3次,共计8周时间。B组采用常规运动疗法方案,包括膝关节和髋关节的稳定性训练、力量训练,专项训练等,每周3次,每次30min,共计8周。C组在B组的基础上加入髋关节的强化训练,每周3次,每次30min,共计8周。训练前后及每周测试受试者的数字疼痛评定量表NPRS,干预前后测试下肢功能量表LEFS、测力计DN,单腿下蹲和Y平衡YBT。结果:与干预前相比,B组和C组的NPRS、LEFS、DN、YBT明显好于干预前,且差异具有统计学意义(P<0.05),干预后三组相比较,B组和C组的NPRS、LEFS、DN、YBT明显好于A组,且差异具有统计学意义(P<0.05)。干预后,进行髋关节强化训练的C组在LEFS、DN、YBT明显好于B组,且差异具有统计学意义(P<0.05)。结论:常规运动疗法和髋关节强化性训练的训东疗法均能有效改善女性跑步者髂胫束综合征,但是髋关节强化运动对于供能改善,下肢力量刚面改善具有更好的效果,值得进一步推广应用。
The incidence of iliotibial band syndrome is very high among runners.The purpose of this study is to explore the effect of hip intensive training on female runners with iliotibial band syndrome,and to provide reference for the exercise therapy of iliotibial band syndrome.Methods:60 female runners with iliotibial band syndrome were randomly divided into iliotibial band stretching group(a,n=20),routine exercise group(B,n=20)and hip joint strengthening exercise group(C,n=20).Group A received iliotibial band stretching training,3 times a day,10 minutes each time and 3 times a week for 8 weeks.Group B received routine exercise therapy,including knee and hip stability training,strength training,special training,3 times a week,30 minutes each time,for a total of 8 weeks.On the basis of group B,group C received hip joint intensive training,3 times a week,30 minutes each time,for a total of 8 weeks.Before and after the training and every week,the subjects’digital pain rating scale(NPRs),lower limb function scale(NPRs),dynamometer(DN),single leg squat and y-balance(YBT)were tested.Results:compared with before intervention,the NPRs,NPRs,DN and YBT of group B and group C were significantly better than those before intervention,and the difference was statistically significant(P<0.05).After intervention,the NPRs,NPRs,DN and YBT of group B and group C were significantly better than those of group A,and the difference was statistically significant(P<0.05).After the intervention,group C was significantly better than group B in NPRs,DN and YBT,and the difference was statistically significant(P<0.05).Conclusion:both conventional exercise therapy and hip joint intensive training can effectively improve iliotibial band syndrome of female runners,but hip joint intensive exercise has better effect on improving energy supply and lower limb strength stiffness,which is worthy of further promotion and application.
