彩色多普勒超声检查对糖尿病下肢动脉病变患者的临床诊断价值

The Value of Color Doppler Ultrasonography in the Clinical Diagnosis of Diabetic Lower Extremity Arterial Disease

目的探讨彩色多普勒超声检查对2型糖尿病(T2DM)下肢动脉病变患者的临床诊断价值。方法选择2020年6月-2021年5月该院收治的66例T2DM患者作为研究组,选择同期于该院进行体检的65名健康体检者作为对照组。比较两组下肢动脉内径、阻力指数与内膜-中层厚度,以及下肢动脉血管病变的检出情况。结果研究组患者空腹血糖、餐后2 h血糖与糖化血红蛋白水平均高于对照组,差异有统计学意义(P<0.05)。研究组胫后动脉、足背动脉的内径低于对照组,差异有统计学意义(P<0.05),股动脉、腘动脉、胫后动脉、足背动脉的中膜-中层厚度与阻力指数高于对照组,差异有统计学意义(P<0.05)。研究组下肢动脉血管病变的检出率(16.67%)高于对照组(3.08%),差异有统计学意义(P<0.05)。结论彩色多普勒超声检查能够有效明确T2DM患者的下肢动脉病变情况,为临床诊疗提供可靠的参考依据,具有显著的临床应用价值。

Objective To explore the clinical diagnostic value of color Doppler ultrasonography in patients with type 2 diabetes mellitus(T2DM)with lower extremity arterial disease.Methods The 66 patients with T2DM admitted to the hospital from June 2020 to May 2021 were selected as the study group,and 65 healthy patients who underwent physical examinations in the hospital during the same period were selected as the control group.The inner diameter,resistance index and intima-media thickness of the lower extremity arteries,and the detection of lower extremity arterial vascular diseases were compared between the two groups.Results The fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin levels of the patients in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).The inner diameters of the posterior tibial artery and dorsal foot artery in the study group were lower than those of the control group,and the difference was statistically significant(P<0.05).The media-media thickness and resistance index of the femoral artery,popliteal artery,posterior tibial artery,and dorsal foot artery were higher than those in the control group,and the difference was statistically significant(P<0.05).The detection rate of lower extremity arterial vascular disease in the study group(16.67%)was higher than the control group(3.08%),and the difference was statistically significant(P<0.05).Conclusion Color Doppler ultrasonography can effectively clarify the lower extremity arterial lesions of T2DM patients,provide reliable reference basis for clinical diagnosis and treatment,and has significant clinical application value.
