摘要
目的研究菌血症采用血清降钙素原(PCT)以及血液细菌培养后的诊断效果。方法选取2019年6月-2020年6月间该院共收取的40例菌血症患者(高度疑似),全部患者给予PCT以及血液细菌培养检测,对其检测结果进行互比分析。结果病理诊断阳性为38例(95.00%),非菌血症为2例(5.00%);血液细菌培养检查中28例患者(70.00%)被确诊,其余12例(30.00%)为非菌血症;PCT检查中36例患者(90.00%)被确诊,其余4例(10.00%)为非菌血症;PCT和血液细菌培养检查结果特异性比较,差异无统计学意义(χ^(2)=0.000,P>0.05),灵敏度以及准确率中PCT检查显著高于血液细菌培养检查,差异有统计学意义(χ^(2)=14.231、14.221,P<0.05);PCT检查方法无论在发热后1 d、发热后2 d亦是发热后3 d阳性率均显著高于血液细菌培养检查,差异有统计学意义(χ^(2)=5.556、9.941、33.00,P<0.05)。结论与血液细菌培养互比,PCT检查方法应用在菌血症中敏感性、特异性、准确性更高,更加适合临床菌血症疾病的检查,故具有更高的临床应用价值。
Objective To study the diagnostic effect of serum procalcitonin and blood bacterial culture for bacteremia.Methods A total of 40 patients with bacteremia(highly suspected)were selected from June 2019 to June 2020 in the hospital.All patients were given PCT and blood bacterial culture tests,and their test results were compared and analyzed.Results The pathological diagnosis was positive in 38 cases(95.00%),non-bacteremia was in 2 cases(5.00%);28 patients(70.00%)were diagnosed in the blood bacterial culture examination,and the remaining 12 cases(30.00%)were non-bacterial blood In the PCT examination,36 patients(90.00%)were diagnosed,and the remaining 4 cases(10.00%)were non-bacteremia;the results of PCT and blood bacterial culture examinations were specifically compared,and the difference was not statistically significant(χ^(2)=0.000,P>0.05),sensitivity and the accuracy rate of PCT examination was significantly higher than that of blood bacterial culture examination,and the difference was statistically significant(χ^(2)=14.231,14.221,P<0.05);the positive rate of PCT examination method both 1 d after fever,2 d after fever or 3 d after fever was significantly higher than the blood bacterial culture examination,and the difference was statistically significant(χ^(2)=5.556,9.941,33.000,P<0.05).Conclusion Compared with blood bacterial culture,the PCT examination method has higher sensitivity and accuracy in bacteremia,and is more suitable for the examination of clinical bacteremia diseases.Therefore,it has higher clinical application value and is worthy of clinical application.
