眼球扫视运动在帕金森病患者中的临床分析 被引量:1

摘要 目的运用视频眼震电图检查帕金森病(PD)患者眼球扫视运动的特点,评价其在PD中的临床价值。方法选取2018年6月-2019年6月在该院治疗的36例PD患者作为观察组,选取同期36名健康人作为对照组,对两组人群进行视频眼震电图(VNG)检查,比较观察组和对照组扫视潜伏期、峰速度、准确度的差异。结果观察组扫视运动潜伏期15°、20°、30°的向左扫视角度分别为(256.1±80.2)、(276.5±63.8)、(295.2±80.3)ms;向右扫视角度分别为(251.3±70.1)、(279.7±73.2)、(291.1±133.7)ms均较对照组延长,差异有统计学意义(t=3.105、3.933,4.654、2.281、3.463、3.059,P<0.05)。观察组扫视运动速度15°、20°、30°的向左扫视角度分别为(242.5±60.6)、(236.8±68.3)、(295.2±80.3)°/s;向右扫视角度分别为(241.2±53.8)、(233.1±74.2)、(297.1±67.7)°/s,均较对照组降低,差异有统计学意义(P=2.266、2.529、4.548、2.561、2.454、4.533,P<0.05)。观察组扫视运动准确度15°、20°、30°的向左扫视角度分别为(100.7±13.9)%、(85.2±25.5)%、(85.8±18.4)%;向右扫视角度分别为(94.7±17.3)%、(88.2±16.6)%、(83.8±21.0)%,均较对照组降低,差异有统计学意义(t=2.350、2.040、2.196、2.169、2.426、2.109,P<0.05)。结论帕金森病患者眼球扫视存在运动障碍,可能有助于PD的早期诊断。 Objective To use video nystagmus to examine the characteristics of saccadic movement in patients with Parkinson's disease(PD),and to evaluate its clinical value in PD.Methods 36 PD patients treated in the hospital from June 2018 to June 2019 were selected as the observation group,36 healthy people in the same period were selected as the control group,and video nystagmus(VNG)examinations were performed on the two groups.Compared the differences in saccade latency,peak velocity,and accuracy between the observation group and the control group.Results The left saccade angles of 15°,20°and 30°in the observation group were(256.1±80.2)ms,(276.5±63.8)ms and(295.2±80.3)ms,respectively;the right saccade angles were(251.3±70.1)ms,(279.7±73.2)ms and(291.1±133.7)ms,respectively,which were significantly longer than those in the control group,the difference was statistically significant(t=3.105,3.933,4.654,2.281,3.463,3.059,P<0.05).The left saccade angles of the observation group with saccade movement speeds of 15°,20°and 30°were(242.5±60.6)°/s,(236.8±68.3)°/s and(295.2±80.3)°/s,respectively;the right saccade angles were(241.2±53.8)°/s,(233.1±74.2)°/s and(297.1±67.7)°/s,respectively,which were significantly lower than those in the control group,the difference was statistically significant(t=2.266,2.529,4.548,2.561,2.454,4.533,P<0.05).The left saccade angles with saccade motion accuracy of 15°,20°and 30°in the observation group were(100.7±13.9)%,(85.2±25.5)%,(85.8±18.4)%,respectively;the right saccade angles were(94.7±17.3)%,(88.2±16.6)%and(83.8±21.0)%,respectively,which were significantly lower than those in the control group,the difference was statistically significant(t=2.350,2.040,2.196,2.169,2.426,2.109,P<0.05).Conclusion Patients with Parkinson's disease have saccadic dyskinesias,which may help the early diagnosis of PD.
