盐酸多奈哌齐联合奥氮平对老年痴呆精神行为障碍患者认知功能的影响

The Effect of Donepezil Hydrochloride Combined with Olanzapine on Cognitive Function in Patients with Senile Dementia and Mental Behavior Disorder

目的分析评价老年痴呆精神行为障碍治疗期间联合应用盐酸多奈哌齐、奥氮平对于患者认知功能产生的影响。方法以2018年6月-2019年12月该院接受治疗的58例老年痴呆精神行为障碍患者展开对照研究,依照随机数表法划分为对照组和观察组,每组29例。对照组患者在治疗期间单独应用奥氮平,观察组以对照组为基础加用盐酸多奈哌齐。对比分析两组患者临床治疗效果,以患者治疗前后认知功能、临床治疗效果、药物不良反应及不同阶段的BEHAVE-AD评分为观察指标。结果和对照组(72.41%)进行比较,观察组临床治疗有效率(93.10%)明显较高,差异有统计学意义(χ^(2)=4.350,P<0.05);药物作用所引起不良反应对比,观察组和对照组比较差异无统计学意义(χ^(2)=0.145,P>0.05)。治疗前,和对照组进行比较,观察组患者相关认知功能评分差异无统计学意义(P>0.05);治疗后,观察组认知功能等相关评分均具有明显优势,差异有统计学意义(P<0.05);治疗后,不同阶段内观察组患者的BEHAVE-AD评分均优于对照组,差异有统计学意义(t=2.192、6.131、7.060,P<0.05)。结论在老年痴呆精神行为障碍患者治疗期间选择联合应用盐酸多奈哌齐、奥氮平对改善患者认知功能、提升临床治疗效率具有确切效果,且药物安全性较高。

Objective To analyze and evaluate the effect of combined use of donepezil hydrochloride and olanzapine on patients'cognitive function during the treatment of senile dementia with mental behavior disorder.Methods A controlled study was conducted with 58 patients with Alzheimer's and mental behavior disorder treated in the hospital from June 2018 to December 2019.They were divided into control group and observation group according to the random number table method,with 29 cases in each group.The patients in the control group were treated with olanzapine alone during the treatment,and the observation group was treated with donepezil hydrochloride based on the control group.The clinical treatment effects of the two groups were compared and analyzed.The patients before and after treatment of cognitive function,clinical treatment effects,drug side effects and BEHAVE-AD scores at different stages were the observation indicators.Results Compared with the control group(72.41%),the effective rate of clinical treatment in the observation group(93.10%)was higher,and the difference was statistically significant(χ^(2)=4.350,P<0.05).There was no statistically significant difference between the control group(χ^(2)=0.145,P>0.05).Before the treatment measures,compared with the control group,there was no statistically significant difference in the cognitive function scores of the observation group patients(P>0.05);after the treatment measures were taken,the observation group's cognition function and other related scores have obvious advantages,and the difference was statistically significant(P<0.05);the BEHAVE-AD scores of patients in the observation group are better than those of the control group at different stages after the treatment measures are taken,and the difference was statistically significant(t=2.192,6.131,7.060,P<0.05).Conclusion The combined application of donepezil hydrochloride and olanzapine during the treatment of patients with senile dementia and mental behavior disorder has a definite effect on improving patients'cognitive function and improving the efficiency of clinical treatment,and the drug has high safety.
