标准大骨瓣减压术治疗重型脑外伤的疗效及对提高患者生存率分析

Efficacy of Standard Large Bone Craniectomy in the Treatment of Severe Brain Injury and Analysis of Improving the Survival Rate of Patients

目的探究重型脑外伤应用标准大骨瓣减压术的临床疗效。方法将该院自2019年9月-2020年9月期间治疗的64例重型脑外伤患者通过随机抽签法分为研究组与参照组,两组各纳入32例,给予参照组行常规骨瓣开颅术治疗,给予研究组行标准大骨瓣减压术治疗,对比两组患者GCS评分、NIHSS评分、生存质量评分及并发症发生情况。结果研究组GCS评分为(11.63±2.08)分高于参照组的(9.64±1.86)分,NIHSS评分(10.56±1.15)分高于参照组的(15.17±1.74)分,生存质量评分为(93.42±2.26)分高于参照组的(87.62±3.95)分,差异有统计学意义(t=4.034、12.503、7.209,P<0.001),研究组并发症发生率6.25%明显低于参照组的25.00%,差异有统计学意义(χ^(2)=4.266,P=0.038)。结论重型脑外伤应用标准大骨瓣减压术疗效显著,可有效改善神经功能,提高生存质量,预后并发症较少,值得应用推广。

Objective To explore the clinical effect of standard large bone craniectomy for severe traumatic brain injury.Methods 64 patients with severe traumatic brain injury treated in the hospital from September 2019 to September 2020 were divided into the study group and the reference group by random drawing.The two groups were enrolled in 32 cases,and the reference group was given conventional craniotomy treatment,and the study group was given standard large bone craniotomy treatment.The GCS score,NIHSS score,quality of life score and complications were compared between the two groups.Results The GCS score of the study group was(11.63±2.08)points higher than the reference group(9.64±1.86)points,and the NIHSS score(10.56±1.15)points were higher than the reference group(15.17±1.74)points,the quality of life score(93.42±2.26)points was higher than the reference group(87.62±3.95)points,the difference was statistically significant(t=4.034,12.503,7.209,P<0.001),and the complication rate 6.25%of the study group was significantly lower than the 25.00% of the reference group,the difference was statistically significant(χ^(2)=4.266,P=0.038).Conclusion The application of standard large bone craniectomy for severe brain injury has a significant effect,which can effectively improve nerve function,improve quality of life,and has fewer prognostic complications.It is worthy of application and promotion.
