腹腔镜辅助阴式全子宫切除术治疗巨大子宫肌瘤的效果探讨

Effect of laparoscopic-assisted total hysterectomy for the treatment of giant uterine myoma

目的 研究腹腔镜辅助阴式全子宫切除术治疗巨大子宫肌瘤的效果。方法 88例巨大子宫肌瘤患者,根据入院编号奇偶性分为研究组(奇数)与对照组(偶数),各44例。研究组接受腹腔镜辅助阴式全子宫切除术治疗,对照组接受传统开腹子宫切除术治疗。比较两组手术相关指标及并发症发生情况。结果 研究组手术时间(100.27±12.56)min长于对照组的(75.39±10.05)min,肛门排气时间(0.68±0.15)d、下床活动时间(1.01±0.43)d、住院时间(4.21±1.05)d短于对照组的(1.41±0.29)、(2.08±0.61)、(7.82±1.58)d,术中出血量(62.74±8.35)ml少于对照组的(89.16±10.62)ml,差异均有统计学意义(P<0.05)。研究组并发症率为4.55%,低于对照组的27.27%,差异有统计学意义(P<0.05)。结论 腹腔镜辅助阴式全子宫切除术治疗巨大子宫肌瘤具有较为理想的临床疗效,且患者恢复更快,没有严重的并发症,是一种兼具有效性与安全性的治疗方式。

Objective To study the effect of laparoscopic-assisted total hysterectomy for the treatment of giant uterine myoma.Methods A total of 88 patients with giant uterine fibroids were divided into research group(odd number)and control group(even number)according to the parity of admission number,with 44 cases in each group.The research group received laparoscopic-assisted total hysterectomy,and the control group still received traditional open hysterectomy.The surgery-related indicators and complication rates were compared between the two groups.Results The operation time(100.27±12.56)min of the research group was longer than(75.39±10.05)min of the control group,the anal exhaust time(0.68±0.15)d,off-bed activity time(1.01±0.43)d,hospitalization time(4.21±1.05)d were shorter than(1.41±0.29),(2.08±0.61),(7.82±1.58)d of the control group,and the intraoperative blood loss(62.74±8.35)ml was less than(89.16±10.62)ml of the control group.All the differences were statistically significant(P<0.05).The complication rate in the research group was 4.55%,which was lower than 27.27%in the control group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic-assisted total hysterectomy for the treatment of giant uterine myoma does have a relatively ideal clinical effect,and the patient recovers faster without serious complications.It is a treatment with both effectiveness and safety.
