卡维地洛治疗老年充血性心力衰竭的效果评价

Evaluation of the effect of carvedilol in the treatment of congestive heart failure in the elderly

目的评价卡维地洛治疗老年充血性心力衰竭(CHF)的临床疗效。方法90例老年CHF患者,随机分为对照组与观察组,每组45例。对照组患者采取吸氧、利尿剂、强心剂等常规治疗,观察组患者在对照组基础上联合卡维地洛治疗。比较两组患者治疗效果、不良反应发生情况及治疗前后心功能指标[左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)]。结果观察组患者治疗总有效率为93.33%,高于对照组的75.56%,差异具有统计学意义(P<0.05);两组患者不良反应发生率比较差异无统计学意义(P>0.05)。治疗3个月后,两组患者LVESD、LVEDD、LVEF均优于本组治疗前,且观察组患者LVESD(43.78±3.50)mm、LVEDD(57.10±4.20)mm、LVEF(49.45±4.30)%优于对照组的(48.05±3.60)mm、(63.50±4.30)mm、(44.72±4.50)%,差异具有统计学意义(P<0.05)。结论常规对症支持辅助卡维地洛治疗老年CHF效果理想、安全性高,能够促进患者心功能改善,值得临床推广应用。

Objective To evaluate the clinical efficacy of carvedilol in the treatment of senile congestive heart failure(CHF).Methods A total of 90 elderly patients with CHF were randomly divided into the control group and the observation group,with 45 cases in each group.Patients in the control group were treated with conventional treatments such as oxygen inhalation,diuretics,and cardiotonic drugs,and patients in the observation group were treated with carvedilol on the basis of the control group.Both groups were compared in terms of thereapeutic effects,occurrence of adverse reactions and cardiac function indicators[left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),left ventricular ejection fraction(LVEF)]before and after treatment.Results The total effective rate of treatment in the observation group was 93.33%,which was higher than 75.56%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).After 3 months of treatment,LVESD,LVEDD,and LVEF of the two groups were better than those of this group before treatment;LVESD(43.78±3.50)mm,LVEDD(57.10±4.20)mm,and LVEF(49.45±4.30)%in the observation group were better than(48.05±3.60)mm,(63.50±4.30)mm,and(44.72±4.50)%in the control group;all the differences were statistically significant(P<0.05).Conclusion Conventional symptomatic support assisted with carvedilol has an ideal effect,high safety in the treatment of elderly CHF,and can promote the improvement of patients’cardiac function.It is worthy of clinical promotion and application.
