霉变饲料诱发马大肠杆菌感染临床诊断及治疗

Clinical Diagnosis and Treatment of E.coli Infection in Equine Induced by Moldy Feed

云南省某马术俱乐部的10匹马因饲喂霉变饲料而发病,临床检查可见10匹马均有不同程度的眼结膜潮红、拉黄色稀粪便、尿液发黄粘稠;采集饲料、粪便、血液样品进行实验室检测确诊。对病马饲料进行黄曲霉毒素检测,结果为阳性;采集血清做血液生化指标检测,结果显示谷丙转氨酶(ALT)、谷草转氨酶(AST)、碱性磷酸酶(ALP)、尿素氮(BUN)和肌酐(CRE)均高出马的正常参考值数倍,表明肝肾功能受到不同程度的损伤;取粪便做细菌分离培养及染色镜检,结果为大肠杆菌;分离培养菌株药敏试验结果对阿米卡星、四环素、链霉素极敏感。结合临床症状及实验室诊断结果,确诊该批马因霉菌毒素中毒与大肠杆菌混合感染,马霉菌毒素导致中毒马发生急慢性肝炎、肾炎,肾功能受损,肝肾机能障碍。使用复方中药制剂治疗,治疗第3天,10匹病马都出现好转的现象,7 d后陆续康复。

Ten Horses in an equestrian club in Yunnan province were sick because of feeding mildewed feed.Clinical examination showed that all the 10 horses had different degree of eye conjunctival flushing,pulling yellow loose stool and yellow and sticky urine feed,stool and blood samples were collected for laboratory testing.Tested positive for aflatoxins in sick horse feed,and serum was collected for biochemical markers,the results showed that the reference values of Alanine transaminase,aspartate transaminase,Alkaline phosphatase,Bun and CRE were several times higher than normal,which indicated that the liver and kidney function were damaged in different degrees The stool samples were isolated and cultured for bacteria,and the results were examined by staining microscope.The isolated and cultured strains were highly sensitive to Amikacin,tetracycline and streptomycin.Combined with clinical symptoms and laboratory diagnosis,it was confirmed that this batch of horses was caused by mycotoxin poisoning and mixed infection of E.coli,which caused acute and chronic hepatitis,nephritis,impaired renal function and hepatic and renal dysfunction.On the 3rd day of treatment,10 sick horses were all improved and recovered after 7 days.
