温肾健脾方治疗腹泻型肠易激综合征(IBS)(脾肾阳虚证)的临床疗效

Clinical Effect of Wenshen Jianpi Prescription on Diarrhea Type Irritable Bowel Syndrome(IBS)(Spleen and Kidney Yang Deficiency Syndrome)

目的探究温肾健脾方治疗腹泻型肠易激综合征(IBS)(脾肾阳虚证)的临床疗效。方法以2019年1月—2021年5月该院消化内科门诊收治的103例腹泻型IBS脾肾阳虚证患者为研究对象,按照用药不同将患者分为对照组52例、治疗组51例,对照组接受西药马来酸曲美布汀片治疗,治疗组在对照组基础上联合接受温肾健脾方治疗,比较两组症候积分、病情严重程度、5-羟色胺(5-HT)水平、总有效率。结果治疗4周后,治疗组主症、次症评分均低于对照组,差异有统计学意义(t=9.488、9.269,P<0.05);治疗组腹痛程度、腹痛频率、腹胀程度、排便满意度、生活干扰评分均低于对照组,差异有统计学意义(t=7.449、5.086、4.983、5.802、6.490,P<0.05);治疗组血清5-HT水平低于对照组,差异有统计学意义(t=7.744,P<0.05);治疗组治疗后总有效率为92.16%,明显高于对照组治疗总有效率76.92%,差异有统计学意义(χ^(2)=4.554,P<0.05)。结论温肾健脾方治疗腹泻型IBS脾肾阳虚证患者可更明显减轻症候积分及病情程度,更有效降低5-HT水平,提升疗效,有良好应用价值。

Objective To explore the clinical efficacy of Wenshen Jianpi prescription in the treatment of diarrhea type irritable bowel syndrome(IBS)(spleen and kidney Yang deficiency syndrome).Methods A total of 103 patients with diarrhea IBS syndrome of spleen and kidney Yang deficiency admitted to the department of gastroenterology outpatient department of the hospital from January 2019 to May 2021 were selected as the objects of this study.According to different medications,the patients were divided into control group 52 cases and treatment group 51 cases.The control group received trimebutin maleate tablet,and the treatment group received warm kidney and Spleen decoction on the basis of the control group.Symptom score,severity,5-ht level and total response rate were compared between the two groups.Results After 4 weeks of treatment,the scores of primary and secondary symptoms in the treatment group were lower than those in the control group,the difference was statistically significant(t=9.488,9.269,P<0.05);the degree of abdominal pain,frequency of abdominal pain,degree of abdominal distention,defecation satisfaction and life disturbance score in the treatment group were lower than those in the control group,the difference was statistically significant(t=7.449,5.086,4.983,5.802,6.490,P<0.05);the serum 5-HT level in treatment group was lower than that in control group,the difference was statistically significant(t=7.744,P<0.05);the total effective rate of the treatment group was 92.16%,which was significantly higher than 76.92%of the control group,the difference was statistically significant(χ^(2)=4.554,P<0.05).Conclusion Wenshen Jianpi prescription can significantly reduce the symptom score and disease degree,effectively reduce the level of 5-HT,improve the curative effect,and has good application value in the treatment of diarrhea IBS patients with deficiency of spleen and kidney Yang syndrome.
