超声乳化联合小梁切除术治疗青光眼合并白内障老年患者的临床疗效分析

Analysis of the Clinical Efficacy of Phacoemulsification Combined with Trabeculectomy in the Treatment of Elderly Patients with Glaucoma and Cataract

目的分析超声乳化联合小梁切除术治疗青光眼合并白内障老年患者的临床疗效。方法选取该院2019年1月—2021年5月收治的青光眼合并老年性白内障患者共56例,随机分组,每组28例。单纯手术组的患者实施小梁切除术治疗,联合手术组采取超声乳化联合小梁切除术治疗。比较两组治疗前后患者视力情况、前房深度、眼压、角膜水肿等并发症发生率。结果治疗后,联合手术组视力水平(0.74±0.13)、前房深度(3.68±0.47)mm高于单纯手术组,眼压(14.19±1.48)mmHg显著低于单纯手术组,差异有统计学意义(t=4.319、6.805、6.537,P<0.05)。联合手术组并发症发生率14.28%高于单纯手术组3.57%,差异无统计学意义(χ^(2)=0.004,P>0.05)。结论超声乳化联合小梁切除术治疗青光眼合并白内障老年患者的临床疗效确切,可有效改善患者病情,改善视力,降低眼压,提高前房深度。

Objective To analyze the clinical efficacy of phacoemulsification combined with trabeculectomy in the treatment of elderly patients with glaucoma and cataract.Methods A total of 56 patients with glaucoma and senile cataract who were admitted to the hospital from January 2019 to May 2021 were enrolled in randomized groups,with 28 cases in each group.Patients in the surgery-only group were treated with trabeculectomy,and the combined surgery group was treated with phacoemulsification combined with trabeculectomy.The visual acuity,anterior chamber depth,intraocular pressure,corneal edema and other complications were compared between the two groups before and after treatment.Results After treatment,the visual acuity level of(0.74±0.13),the anterior chamber depth of the combined surgery group was(3.68±0.47)mm,higher than that of the surgery alone group,and the intraocular pressure of(14.19±1.48)mmHg was significantly lower than that of the surgery alone group,the difference was statistically significant(t=4.319,6.805,6.537,P<0.05).The complication rate of the combined operation group(14.28%)was higher than that of the simple operation group(3.57%),the difference was not statistically significant(χ^(2)=0.004,P>0.05).Conclusion Phacoemulsification combined with trabeculectomy has a definite clinical effect in the treatment of elderly patients with glaucoma and cataract.It can effectively improve the patient's condition,improve vision,reduce intraocular pressure,and increase the depth of the anterior chamber.
