NBI放大内镜、染色内镜及白光内镜在结直肠微小息肉中的诊断差异

Comparison of NBI magnifying endoscopy,staining endoscopy and white light endoscopy in the diagnosis of colorectal micro polyp

目的研究窄带成像技术(NBI)放大内镜、染色内镜及白光内镜在结直肠微小息肉中的诊断差异。方法选择结直肠微小息肉患者126例,分别行NBI放大内镜(46例)、染色内镜(40例)和白光内镜(40例)。比较各组结直肠微小息肉检出率、病理结果、微小息肉的诊断效能。结果NBI组对微小息肉的检出显著高于染色内镜组和白光内镜组(P=0.031);各组微小息肉病理诊断结果的差异无显著性(P>0.05);NBI放大内镜对于微小息肉的诊断灵敏度和ROC曲线下面积显著高于其他两种诊断方法。结论NBI放大内镜在结直肠微小息肉中的诊断灵敏度明显高于染色内镜和白光内镜,便于临床推广。

Aim To investigate the differences of narrow band imaging(NBI),staining endoscopy and white light endoscopy in the diagnosis of colorectal micro polyp.Methods 126 cases of patients with colorectal micro polyp diagnosed were selected,46 cases were examined by NBI magnifying endoscopy,40 cases examined by staining endoscopy and 40 cases by white light endoscopy.The difference of detection rate,pathological results,diagnostic efficacy of colorectal micro polyp and post diagnosis efficacy was compared.Results The detection of micro polyps in NBI group was significantly higher than that in staining group and white light group(P=0.031);there was no significant difference in the diagnosis of tumor polyps and non tumor polyps among the three groups(P>0.05);the diagnostic sensitivity of NBI for micro polyps was significantly higher than that of the other two groups.Through the ROC curve analysis,the area under the ROC curve of NBI examination was significantly higher than that of the other two groups.Conclusion Compared with dye endoscopy and white light endoscopy,the sensitivity of NBI magnifying endoscopy in the diagnosis of colorectal micro polyp is significantly improved,which is convenient for clinical promotion.
