CEACAM5和CEACAM6在结肠癌中的表达及临床意义

目的分析癌胚抗原相关细胞粘附分子5(CEACAM5)和癌胚抗原相关细胞粘附分子6(CEACAM6)在结肠癌组织中的表达水平。方法选取本院2012年2月至2016年5月收治的结肠癌患者106例作为研究对象,所有患者均行结肠癌根治术,术中取患者癌旁>5 cm正常组织及结肠癌组织,检测结肠癌组织及癌旁正常组织中CEACAM5和CEACAM6 mRNA表达水平及蛋白表达水平,分析其表达水平与结肠癌患者临床特征及预后的关系。结果结肠癌组织中CEACAM5和CEACAM6 mRNA表达水平均显著高于癌旁正常组织(P<0.05);CEACAM5、CEACAM6蛋白在结肠癌组织中阳性表达率均明显高于癌旁正常组织(P<0.05);结肠癌组织中CEACAM5、CEACAM6表达与结肠癌患者肿瘤分期、肿瘤大小、淋巴结转移和浸润深度有关(P<0.05);CEACAM5阴性表达患者无进展生存率和总生存率均显著高于阳性表达患者(P<0.05);CEACAM6阴性表达患者无进展生存率和总生存率均显著高于阳性表达患者(P<0.05);COX分析显示,CEACAM5、CEACAM6表达水平及浸润深度是影响结肠癌患者预后的独立危险因素(P<0.05)。结论CEACAM5和CEACAM6在结肠癌组织中表达上调,影响患者术后3年的生存率,是影响结肠癌患者预后的独立危险因素,检测其表达水平可作为结肠癌患者疾病发生发展和预后的参考指标。

Objective To analyze the expression levels of carcinoembryonic antigen-related cell adhesion molecule 5(CEACAM5)and carcinoembryonic antigen-related cell adhesion molecule 6(CEACAM6)in colon cancer tissues.Methods 106 patients with colon cancer admitted to our hospital from February 2012 to May 2016 were selected as the research subjects.All patients underwent radical resection of colon cancer.During the operation,normal tissues>5 cm adjacent to the cancer and colon cancer tissues were collected for detection.The mRNA and protein expression levels of CEACAM5 and CEACAM6 in colon cancer tissues and adjacent normal tissues were detected,and the analyzed relationship between their expression levels and the clinical characteristics and prognosis of patients with colon cancer.Results The expression levels of CEACAM5 and CEACAM6 mRNA in colon cancer tissues were significantly higher than those in adjacent tissues(P<0.05);the positive expression rates of CEACAM5 and CEACAM6 proteins in colon cancer tissues were significantly higher than those in normal adjacent tissues(P<0.05);the expression of CEACAM5 and CEACAM6 in cancer tissues was related to tumor stage,tumor size,lymph node metastasis and depth of invasion in colon cancer patients(P<0.05);the progression-free survival rate and overall survival rate of patients with negative expression of CEACAM5 were significantly higher than those with positive expression(P<0.05);the progression-free survival rate and overall survival rate of patients with negative expression of CEACAM6 were significantly higher than those of positive expression(P<0.05);COX analysis showed that CEACAM5,CEACAM6 expression level and depth of invasion were independent risks affecting the prognosis of colon cancer patients factors(P<0.05).Conclusion The expressions of CEACAM5 and CEACAM6 are up-regulated in colon cancer tissues,which affect the postoperative 3-year survival rate of patients,they are independent risk factors affecting the prognosis of patients with colon cancer,detection of CEACAM5 and CEACAM6 their expression levels can be used as reference index for the occurrence,development,and prognosis of patients with colon cancer.
