摘要
目的探讨纤维蛋白降解产物(FDP)、外周血中性粒细胞与淋巴细胞比值(NLR)和单核细胞与淋巴细胞比值(MLR)与肺癌预后的相关性。方法选取2018年1月至2020年11月本院收治的80例晚期肺癌患者作为研究组,根据患者预后结局分为生存组(n=55)与死亡组(n=25),另选取同期80例健康体检者作为对照组。比较不同组别、不同肺癌预后FDP、NLR和MLR水平,绘制受试者工作特征(ROC)曲线,分析FDP、NLR和MLR预测肺癌预后的价值。结果研究组FDP、NLR和MLR水平均高于对照组,差异有统计学意义(P<0.05);死亡组FDP、NLR和MLR水平均高于生存组,差异有统计学意义(P<0.05);FDP测肺癌预后的AUC为0.581,特异度和敏感度分别为26.21%、92.15%,NLR预测肺癌预后的AUC为0.785,特异度和敏感度分别为26.21%、92.15%;MLR预测肺癌预后的AUC为0.538,特异度和敏感度分别为80.21%、31.32%。结论FDP、NLR和MLR均为诊断肺癌的有效手段,可预测肺癌预后。
Objective To investigate the correlation between fibrin degradation products(FDP),peripheral blood neutrophil to lymphocyte ratio(NLR)and monocyte to lymphocyte ratio(MLR)and the prognosis of lung cancer.Methods 80 patients with advanced lung cancer admitted to our hospital from January 2018 to November 2020 were selected as the study group,they were divided into survival group(n=55)and death group(n=25)according to prognosis outcome,and 80 healthy people in the same period were selected as the control group.The FDP,NLR and MLR levels in different groups and different lung cancer prognosis were compared,draw receiver operating characteristic(ROC)curve,and analyzed the prognostic value of FDP,NLR and MLR in advanced lung cancer.Results The levels of FDP,NLR and MLR in the study group were higher than those in the control group,the difference was statistically significant(P<0.05);the FDP,NLR and MLR levels in the death group were higher than those in the survival group,the difference was statistically significant(P<0.05);the AUC of FDP for the prognosis of advanced lung cancer was 0.581,and the specificity and sensitivity were 26.21%and 92.15%,respectively.The AUC for predicting the prognosis of advanced lung cancer by NLR was 0.785,and the specificity and sensitivity were 26.21%and 92.15%,respectively.The AUC for predicting the prognosis of advanced lung cancer by MLR was 0.538,and the specificity and sensitivity were 80.21%and 31.32%,respectively.Conclusion FDP,NLR and MLR are all effective methods for the diagnosis of advanced lung cancer and can predict the prognosis of advanced lung cancer.
