超声导向在绝经后女性宫内节育器(IUD)取出术中的应用探讨

Application of ultrasound guidance in removal of intrauterine device(IUD)in postmenopausal women

目的探究超声导向在绝经后女性宫内节育器(IUD)取出术中的应用效果。方法选取本院2018年1月至2020年11月收治的472例绝经后女性作为研究对象,按照随机数字表法分为对照组与观察组,每组236例。对照组实施传统IUD取出术,观察组在超声引导下进行IUD取出手术。比较两组围术期相关指标、疼痛评分、应激指标、一次性IUD取出成功率、不良事件发生率。结果观察组术中出血量少于对照组,手术时间、术后疼痛持续时间、术后阴道出血持续时间均短于对照组,差异有统计学意义(P<0.05);术中、术后3 d,观察组疼痛评分低于对照组,AngⅡ、Cor、NE水平均低于对照组,差异有统计学意义(P<0.05);观察组一次性取出IUD成功率高于对照组,不良事件发生率低于对照组,差异有统计学意义(P<0.05)。结论超声导向在绝经后女性宫内节育器取出术中的应用效果显著,应激反应、疼痛感均较轻,且一次性取出IUD成功率高,具有较高的安全性,值得临床推广应用。

Objective To explore the effect of ultrasound guidance in the removal of intrauterine device(IUD)in postmenopausal women.Methods 472 postmenopausal women admitted to our hospital from January 2018 to November 2020 were selected as the research subjects,and they were divided into control group and observation group according to random number table method,with 236 cases in each group.The control group underwent traditional IUD removal surgery,and the observation group underwent IUD removal surgery under ultrasound guidance.The Perioperative related indexes,pain score,stress index,one-time IUD removal success rate and incidence of adverse events were compared between the two groups.Results The intraoperative blood loss of the observation group was less than that of the control group,and the operation time,postoperative pain duration and postoperative vaginal bleeding duration were shorter than those of the control group,the difference was statistically significant(P<0.05);during and 3 days after operation,the pain score of the observation group was lower than that of the control group,and the levels of AngⅡ,Cor and NE were lower than those of the control group,the difference was statistically significant(P<0.05).The success rate of one-time IUD removal was higher than that of the control group,and the incidence of adverse events was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion Ultrasound guidance is effective in the removal of IUD in postmenopausal women,the stress response and pain are less,and the success rate of IUD removal at one time is high,it has high safety and is worthy of clinical application.
