Professor Yang Qinhe's experience in treating Hepatitis B Cirrhosis in Lingnan area

慢性乙型肝炎后肝硬化是临床最常见的慢性肝病之一,在岭南地区较为多发,其临床表现具有地域特征。杨钦河教授强调岭南地区乙肝后肝硬化多以湿热疫毒侵袭,肝失条达,脾不健运,湿热蕴结,气滞血瘀,邪毒久羁,肝肾亏虚为其病因病机。在临床治疗该病的过程中,杨教授强调中西医结合,西药以抗病毒治疗为主,中医治疗方面重视疾病的分期论治,注重标本兼顾,调燮脾胃,扶正驱邪;在疾病早期清利湿热为主,兼以疏肝健脾、软坚散结;在疾病中期活血化瘀、软坚散结并重,兼以清热调肝健脾;在疾病后期以健脾养肝补肾为主,活血化瘀、软坚散结为辅。同时重视以人为本的理念,叮嘱患者要调畅情志,注意起居饮食和劳倦等。

Hepatitis B Cirrhosis(HBC)is one of the most common chronic liver diseases,which is common in Lingnan area,and its manifestations have regional characteristics.Professor Yang Qinhe pointed out that the etiology and pathogenesis of HBC in Lingnan area is mostly that it is caused by invasion of damp-heat and epidemic virus,then the liver lost stretch,the spleen lost its normal functioning,the damp and heat accumulates,the Qi stagnation and the blood stasis,finally lead to deficiency of liver and kidney function.For the treatment of this disease,Professor Yang emphasized treatment with a combination of Chinese and Western medicine,the main western medicine are antiviral drugs,for chinese medicine treatment,Professor Yang paid attention to the staged treatment of the disease,focused on regulating the spleen and stomach,strengthening the body and exorcising evil.In the early stage of the disease,the main purpose is to clear away damp-heat,and also to relieve the liver and invigorate the spleen,soften the firmness and dispel masses.In the middle stage of the disease,the main purpose is to promote blood circulation and remove blood stasis,soften the firmness and dispel masses,and simultaneously clear heat,regulate liver and spleen.In the later stage of the disease,the main purpose is to strengthen the spleen,nourish the liver and the kidney,and simultaneously promote blood circulation and remove blood stasis,soften the firmness and dispel masses.Meanwhile,Professor Yang emphasized the people-oriented concept,instruct patients to adjust their emotions,pay attention to daily life,diet and fatigue.
