微生态制剂的补充对川崎病的炎性指标的影响

摘要 目的探讨微生态制剂的补充对川崎病(KD)的临床疗效及炎性指标的影响。方法选择2018年1月—2020年12月本院收治的68例KD患儿作为研究对象,随机数表法分为实验组和对照组两组,每组各34例。对照组予以常规治疗,实验组在常规治疗基础上加用双歧杆菌四联活菌片,治疗2周后观察两组临床症状改善情况、血清炎症指标变化及冠状动脉损害发生情况。结果实验组发热、黏膜弥漫性充血、颈淋巴结肿大、躯干红斑等临床症状消退时间及住院时间明显短于对照组,差异均有统计学意义(P<0.05);两组入院时外周血WBC、PLT、ESR、CRP水平比较,差异均无统计学意义(P>0.05),治疗结束两组上述指标均明显降低(P<0.05),且实验组各指标降低幅度大于对照组(P<0.05);实验组治疗后1个月冠状动脉损伤总发生率为5.88%,明显低于对照组的14.71%,且冠状动脉损伤程度轻于对照组(P<0.05)。结论微生物制剂补充可改善KD患儿临床症状,降低冠状动脉损害并发风险,并能有效控制机体炎症反应,可作为KD临床治疗的辅助用药,值得临床推广。 Objective To explore the effect of supplementation of microecological preparations on the clinical efficacy and inflammatory indexes of Kawasaki disease(KD).Methods 68 patients with KD who were hospitalized in our hospital from January 2018 to December 2020 were selected as study objects.The children were randomly divided into 2 groups of experimental group and control group according to random number table method,34 cases in each group.The control group was given conventional treatment,and the experimental group was treated with Bifidobacterium quadruple viable tablets on the basis of conventional treatment.After 2 weeks of treatment,the improvement of clinical symptoms,changes of serum inflammatory indicators and the occurrence of coronary artery in the two groupswere observed.Results The time of clinical symptoms disappear such as fever,diffuse mucosal hyperemia,cervical lymphadenopathy,trunk erythema in the test group and the time of hospitalization were significantly shorter than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the levels of peripheral blood WBC,PLT,ESR,CRPbetween the two groups at the time of admission(P>0.05).At the end of the treatment,the above indicators of the two groups were significantly reduced(P<0.05),and the reduction of each index in the experimental group was greater than that in the control group(P<0.05).The total incidence of coronary artery injury in the experimental group was 5.88%after 1 months of treatment,which was significantly lower than that in the control group(14.71%),and the degree of coronary artery injury was mildcompared with that in the control group(P<0.05).Conclusions Microbial supplementation could improve the clinical symptoms of children with KD,reduce the risk of coronary artery damage,and could effectively control the inflammatory response.It could be used as an auxiliary drug for the clinical treatment of KD and is worthy of clinical promotion.
