带孔克氏针辅助强力非吸收外科缝线技术治疗肘关节恐怖三联征

The treatment for the terrible triad of elbow joint using Kirschner wire with hole assistedhigh-tenacity non-absorbable surgical suture technique

目的讨论带孔克氏针辅助强力非吸收外科缝线技术手术治疗肘关节恐怖三联征的效果。方法回顾性分析2016年1月—2020年3月本院收治的符合随访研究的26例手术患者的临床资料,桡骨头骨折Mason分型,Ⅰ型2例,Ⅱ型10例,Ⅲ型14例;冠状突骨折Regan-Morrey分型,Ⅰ型5例,Ⅱ型15例,Ⅲ型6例。各患者均以带孔克氏针辅助强力非吸收外科缝线技术为基础实施手术,随访26例纳入标准的患者术后肘关节功能评分、手术优良率及并发症情况。结果术后至少随访6-12个月,平均(10.04±2.23)个月。末次随访的肘关节功能评分为(85.62±9.83)分,手术优良率为84.62%(22/26)。出现关节僵硬2例、异位骨化2例,无感染、脱位、骨折不愈合等。结论使用带孔克氏针辅助强力非吸收外科缝线技术固定冠状突骨折,固定了冠状突骨折块,修复了肘关节前囊,术后效果满意。

Objective To evaluate the clinical effect of kirschner wire with hole assisted high-tenacity non-absorbable surgical suture technique in the treatment of terrible triad of elbow joint.Methods The clinical data of 26 patients conform to follow-up studythose were treated in our hospital from January 2016 to March 2020 were retrospectively analyzed.According to the Mason classification, there were 2 cases of type Ⅰ,10 cases of type Ⅱ and 14 cases of type Ⅲ;according to Regan-Morrey classification for fracture of coronoid process, there were 5 cases of type Ⅰ,15 cases of type Ⅱ and 6 cases of type Ⅲ.All patients were operated on the basis of kirschner wire with hole and high-tenacity non-absorbable surgical suture technique.The elbow joint function score, excellent and good rate of operation and complications of the enrolled 26 patients were recorded.Results All patients were followed up for 6 to 12 months(10.04±2.23)month on average.At the last follow-up, the elbow function score was(85.62±9.83),and the excellent and good rate was 84.62%(22/26).There were 2 patients developed heterotopic ossification and 2 patients developed joint stiffness after surgery.There were no complications such as infection, nonunion of fracture and dislocation.Conclusions The application of kirschner wire with hole and high-tenacity non-absorbable surgical suture techniquein the fixation of fracture of coronoid process, fixed the fracture mass of the coronoid process, repaired the anterior capsule of the elbow, achieved satisfactory postoperative effect.
