摘要
目的探寻多模式呼吸训练对磁共振成像(MRI)钆塞酸二钠(Gd-EOB-DTPA)肝脏肿瘤筛查增强扫描的检查时间、图像质量、短暂性剧烈呼吸运动(TSM)等的影响。方法选取在我院行MRI钆塞酸二钠早期肝脏肿瘤筛查增强扫描的受检者783例,按呼吸训练模式分为试验组和对照组;试验组560例受检者进行三种不同模式的呼吸训练,即方法A:自主吸气-呼气-屏气263例;方法B:自助吸气-呼气-屏气207例;方法C:他人协助吸气-呼气-屏气90例;对照组223例受检者按常规检查过程中临时语音提示训练;比较两组受检者之间安全性、检查时长、图像质量及短暂性剧烈呼吸运动发生率,检验水准α=0.05。结果试验组受检者的检查时间长短和图像质量比均优越于对照组(P<0.05);试验组与对照组间对比剂外渗、不良反应及短暂性剧烈呼吸运动发生率差异无统计学意义(P>0.05)。结论MRI钆塞酸二钠早期肝脏肿瘤筛查增强扫描安全性高,该检查时间、图像质量与受检者吸气-呼气-屏气配合有关,多模式呼吸训练有利于缩短检查时间,提高图像质量;值得临床影像科室借鉴。
Objective To investigate the effects of multi-mode breathing training on the examination time,image quality and transient acute respiratory movement(TSM)of MRI(Gd-EOB-DTPA)scan for liver tumor screening.Methods 783 subjects who underwent MRI(Gd-EOB-DTPA)enhancement scan for early liver tumor screening in our hospital were selected and divided into experimental and control groups according to the mode of breathing training;560 patients in the test group were trained in three different modes of breathing:method A:self inhalation,exhalation and breath holding(263 patients);method B:self inhalation,exhalation and screen(207 cases);method C:inhalation,exhalation and breath holding with assistance of others(90 cases).223 cases in the control group were trained according to the temporary voice prompt during the routine examination.The safety,examination time,image quality and the incidence of transient severe breathing exercise were compared between the two groups,and the examination level wasα=0.05.Results The inspection time and image quality ratio of the subjects in the test group were superior to those in the control group(P<0.05);there was no significant difference in the incidence of contrast extravasation,adverse reactions and transient violent respiratory movements between the experimental and control groups(P>0.05).Conclusion MRI Gd-EOB-DTPA enhanced scan for early liver tumor is safe.The examination time and image quality are related to the subjects'inhalation-exhalation-holding breath,and multi-modal breathing training is beneficial to shorten the examination time and improve image quality.It is worthy of reference for clinical imaging departments.
