奥曲肽联合深度水解配方奶粉在新生儿坏死性小肠结肠炎中的应用效果及对血清PA、MTL水平的影响

Application effect of octreotide combined with extensively hydrolyzed formula in neonatal necrotizing enterocolitis and its effect on serum PA and MTL levels

目的观察奥曲肽联合深度水解配方奶粉在新生儿坏死性小肠结肠炎(NEC)中的应用效果。方法选取2015年1月至2017年1月我院86例NEC患儿,按随机数表法将其分为研究组和对照组,各43例。研究组给予奥曲肽+深度水解配方奶粉,对照组给予奥曲肽+常规配方奶粉。比较两组的干预效果。结果研究组的治疗总有效率及治疗7 d后体质量高于对照组,喂养不耐受次数少于对照组,住院时间短于对照组,差异具有统计学意义(P<0.05)。治疗7 d后,两组的血清PA、MTL、IgG、IgA、IgM水平及外周血CD4^(+)均较治疗前升高,外周血CD8^(+)均较治疗前降低,且研究组优于对照组,差异具有统计学意义(P<0.05)。结论奥曲肽联合深度水解配方奶粉应用于NEC患儿中,可提升治疗效果,有助于改善其营养状态、胃肠功能及免疫功能。

Objective To observe the application effect of octreotide combined with extensively hydrolyzed formula in neonatal necrotizing enterocolitis(NEC).Methods Eighty-six children with NEC in our hospital from January 2015 to January 2017 were selected and divided into study group and control group according to the random number table method,with 43 cases in each group.The study group was given octreotide+extensively hydrolyzed formula,and the control group was given octreotide+conventional formula.The intervention effects of the two groups were compared.Results The total effective rate of treatment and body mass after 7 d of treatment of the study group were higher than those of the control group,the times of feeding intolerance was less than that of the control group,the hospital stay was shorter than that of the control group,and the differences were statistically significant(P<0.05).After 7 d of treatment,the levels of serum PA,MTL,IgG,IgA,IgM and peripheral blood CD4+in the two groups were higher than those before treatment,the peripheral blood CD8+was lower than that before treatment,those in the study group were better than the control group,and the differences were statistically significant(P<0.05).Conclusion Octreotide combined with extensively hydrolyzed formula for NEC children can improve the therapeutic effect and help to improve their nutritional status,gastrointestinal function and immune function.
