DCE-MRI定量分析对正常卵巢及囊实性卵巢肿瘤的诊断价值研究

Diagnostic Value of DCE-MRI Quantitative Analysis in Normal Ovary and Cystic Solid Ovarian Tumors

目的:探究动态增强磁共振成像(DCE-MRI)定量分析对正常卵巢及囊实性卵巢肿瘤的诊断价值。方法:采集2019年1月~2021年1月本院收治盆腔肿块患者100例临床资料进行回顾性分析。所有患者均接受扩散加权成像(DWI)与DCE-MRI检查,对其检查结果进行统计,并以病理检查结果为金标准,将证实为正常卵巢的患者纳入对照组,将证实为囊实性卵巢肿瘤的纳入观察组,分析两组DWI与DCE-MRI检查结果,评价DWI与DCE-MRI检查对囊实性卵巢肿瘤的诊断效能。结果:100例研究对象经病理检查证实正常卵巢45例,囊实性卵巢肿瘤55例;DWI与DCE-MRI检查显示观察组表观弥散系数、对比剂容积转换常数、速率常数、血管外细胞外间隙百分比均低于对照组;DWI联合DCE-MRI诊断的灵敏度(96.36%)、特异度(97.78%)、准确度(97.00%)、阳性预测值(98.15%)、阴性预测(95.65%)值较DWI(69.09%、86.67%、77.00%、86.36%、69.64%)与DCE-MRI(85.45%、91.11%、88.00%、92.16%、83.67%)高。结论:相对于DWI而言,DCE-MRI定量分析对正常卵巢及囊实性卵巢肿瘤的诊断价值更高,将DWI与DCE-MRI结合应用,可进一步提高临床诊断灵敏度、特异度、准确度,为囊实性卵巢肿瘤治疗提供可靠依据。

Objective:To explore the diagnostic value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)quantitative analysis in normal ovary and cystic solid ovarian tumors.Methods:The clinical data of 100 patients with pelvic masses in our hospital from January 2019 to January 2021 were retrospectively analyzed.All patients received diffusion-weighted imaging(DWI)and DCE-MRI examination,and the examination results were statistically analyzed.The patients confirmed as normal ovaries were included in the control group,and those confirmed as cystic and solid ovarian tumors were included in the study group,objective to evaluate the diagnostic efficacy of DWI and DCE-MRI in cystic and solid ovarian tumors.Results:There were 45 cases of normal ovaries and 55 cases of cystic and solid ovarian tumors;DWI and DCE-MRI showed that the apparent diffusion coefficient,contrast medium volume conversion constant,velocity constant and the percentage of extracellular space in the study group were lower than those in the control group;The sensitivity(96.36%),specificity(97.78%),accuracy(97.00%),positive predictive value(98.15%)and negative predictive value(95.65%)of DWI combined with DCE-MRI were higher than those of DWI(69.09%,86.67%,77.00%,86.36%,69.64%)and DCE-MRI(85.45%,91.11%,88.00%,92.16%,83.67%).Conclusion:Compared with DWI,DCE-MRI quantitative analysis is more valuable in the diagnosis of normal ovary and cystic and solid ovarian tumors.The combination of DWI and DCE-MRI can further improve the sensitivity,specificity and accuracy of clinical diagnosis,and provide reliable basis for the treatment of cystic and solid ovarian tumors.
