论“调气和阴,祛瘀解毒”在慢性萎缩性胃炎治疗中的重要作用

The Important Role of “Regulating Qi and Harmonizing Yin, Removing Blood Stasis and Clearing Away Toxin” Therapy in the Treatment of Chronic Atrophic Gastritis

[目的]探讨中医药治疗慢性萎缩性胃炎的用药规律,进一步提高临床诊治水平。[方法]检索Cochrane Library、PubMed、EMBASE、维普中文期刊数据库、中国知网,中国生物医学文献服务系统、万方数据知识服务平台等数据库,各个数据库的检索时间范围均为建库时间至2021年1月,收集中医药治疗慢性萎缩性胃炎的随机对照试验中有效的方剂,运用数据挖掘(频数分析、共现分析、聚类分析、复杂网络分析、凝聚子群派系分析)进行分析。[结果]共收集文献60篇,处方95首。频数分析发现高频药物33味,以补气理气、解毒化瘀药物为多见;共现分析发现补阴组、益气组、补血组、化痰组、疏肝组等多个药物组合;聚类分析发现7个药物群;复杂网络分析发现白术、白芍为核心药物;凝聚子群派系分析提示以甘草、白芍、白术、黄芪、丹参、当归、半夏、党参8味为共同药物。[结论]"调气和阴、祛瘀解毒"在慢性萎缩性胃炎的治疗中占有重要地位。

[Objective] To explore the medication law of traditional Chinese medicine(TCM) treatment of chronic atrophic gastritis(CAG),and further improve the level of clinical diagnosis and treatment. [Methods] The research collected effective prescriptions demonstrated by randomized controlled trials(RCTs) of TCM treatment of CAG. And data mining(frequency analysis,co-occurrence analysis,cluster analysis,complex network analysis,subgroup faction analysis) was used for analysis. [Results]A total of 60 literatures and 95 prescriptions were collected. Thirty-three kinds of high-frequency herbs were found by frequency analysis. And most of them had effect of invigorating Qi and regulating Qi,removing blood stasis and clearing away toxin. By co-occurrence analysis,multiple herb combinations were found such as invigorating Yin group,invigorating Qi group,tonifying blood group,reducing phlegm group,and soothing the liver group. By cluster analysis,seven types of herb clustering were found. The complex network was centered on Atractylodes Macrocephala Koidz and Paeoniae Radix Alba,and the subgroup faction was in common with licorice,Paeoniae Radix Alba,Atractylodes Macrocephala Koidz,Hedysarum Multijugum Maxim,Radix Salviae,Angelicae Sinensis Radix,Arum Ternatum Thunb and Codonopsis Radix. [Conclusion]"Regulating Qi and harmonizing Yin,removing blood stasis and clearing away toxin"was one of the fundamental treatments for CAG.
