摘要
目的探讨COPD患者血清锌水平及其与CAT评分的关系,为COPD急性加重期临床治疗提供参考。方法选取COPD急性加重期患者51例(COPD急性加重期组),与同期住院无急慢性呼吸系统病史和其他系统感染性疾病史的志愿者(对照组)血清锌含量比较;同时给予常规治疗+补锌治疗至稳定期后(病情基本恢复到急性加重期前的状态超过2周,符合COPD稳定期标准的患者为COPD稳定期组)观察血清锌含量与CAT得分及二者相关性。结果COPD急性加重期组锌缺乏率为39.22%,明显高于对照组(χ^(2)=13.580,P<0.001);血清锌浓度中位数为81.46(56.56,109.51)μg·dL^(-1),明显低于对照组的106.96(92.48,125.92)μg·dL^(-1)(Z=5.529,P<0.001)。稳定期组血清锌水平明显高于急性加重期组(Z=3.692,P<0.001),CAT得分也显著低于急性加重期组(t=7.026,P<0.001),且血清锌水平与CAT得分显著负相关(r=-0.671,-0.323,P<0.05)。结论COPD患者血清锌水平与CAT得分关系密切,在急性加重期更为突出。
Objective To explored the relationship between serum zinc level and CAT score in COPD patients,and provide reference for clinical treatment of acute exacerbation of COPD.Methods Serum zinc content of 51 patients with acute exacerbation of COPD(COPD acute exacerbation group)was compared with that of hospitalized volunteers without history of acute and chronic respiratory system and other systemic infectious diseases(control group)during the same period;at the same time,routine+zinc supplement treatment was given.After the stable period(the condition basically recovered to the state before acute exacerbation for more than 2 weeks.the patients who meet the criteria of COPD stable stage are COPD stable stage group),serum zinc content and CAT score and their correlation were observed.Results The rate of zinc deficiency in the COPD acute exacerbation group was 39.22%,which was significantly higher than that of control group(χ^(2)=13.580,P<0.001);the median of serum zinc level of COPD acute exacerbation group and control group were 81.46(56.56,109.51)μg·dL^(-1),106.96(92.48,125.92)μg·dL^(-1),respectively,the median of serum zinc level of COPD acute exacerbation group was significantly lower than that of control group(Z=5.529,P<0.001).Serum zinc level in the stable period was significantly higher than that of the acute exacerbation period(Z=3.692,P<0.001),CAT score was also significantly lower than that of the acute exacerbation period(t=7.026,P<0.001),and serum zinc level was significantly negatively correlation with CAT score(r=-0.671,-0.323,P<0.05).Conclusion Serum zinc level was significantly correlated with CAT score in COPD patients,especially during acute exacerbation phase.
