血清类黏蛋白1样蛋白质3基因多态性与急性下呼吸道感染的关联性 被引量:2

Polymorphisms of serum cadherin-related family member 3 gene in patients with acute lower respiratory tract infection

摘要 目的探究血清类黏蛋白1样蛋白质3(CDHR3)基因多态性与急性下呼吸道感染(ALRI)的关联性。方法将湖州中心医院2019年1月-2020年12月呼吸与危重症医学科收治的152例ALRI患者作为研究对象,对患者进行CDHR3基因扩增后分析rs6967330位点基因型,按rs6967330位点基因型分为AA、AG和GG三组,比较不同基因型患者的临床特征及实验室指标检测结果差异。结果 ALRI患者中,AA基因型占比最高,为84例占55.26%,同时携带A型基因的患者高于G基因。AA基因型及AG基因型患者出现喘息症状,患支气管肺炎、大叶性肺炎及间质性肺炎的风险更高,组间对比有统计学意义(P<0.05)。三组患者白细胞计数、中性粒细胞、血小板计数指标对比无统计学差异,AA基因型患者CRP水平高于GG基因型患者,组间对比有统计学意义(P<0.05)。结论 CDHR3基因单核苷酸多态性与ALRI产生存在明显联系,提示携带A基因型患者患病风险及病情加重风险相对更高,对患者进行基因分型检测有利于预测患者患病情况为临床治疗提供借鉴信息。 OBJECTIVE To explore the polymorphisms of serum cadherin-related family member 3(CDHR3) gene in patients with acute lower respiratory tract infection(ALRI). METHODS A total of 152 patients with ALRI who were treated in department of respiratory and critical care medicine of Huzhou Central Hospital from Jan 2019 to Dec 2020 were recruited as the study subjects. The CDHR3 gene was amplified, the genotypes at rs6967330 locus were analyzed, the patients were divided into the AA group, the AG group and the GG group according to the genotypes at rs6967330 locus, and the clinical characteristics and laboratory test indexes were observed and compared among the three groups of patients. RESULTS Among the patients with ALRI, the patients with genotype AA accounted for 55.26%(84 cases), and the proportion of the patients with type A gene was significantly higher than that of the patients with type G gene. The patients with genotype AA and genotype AG were at significantly higher risk of wheezing symptoms, bronchopneumonia, lobar pneumonia and interstitial pneumonia(P<0.05). There were no significant differences in the white blood cell counts, neutrophil counts and blood platelet counts among the three groups of patients. The CRP level of the patients with genotype AA and genotype AG was higher than that of the patients with genotype GG, and the CRP level of the patients with genotype AG was significantly higher than that of the patients with genotype GG(P<0.05). CONCLUSION There is significant association between the single nucleotide polymorphisms of CDHR3 gene and the ALRI, which indicates that the patients carrying with genotype A are at higher risk of disease and aggravation of illness condition. The genotyping may facilitates the prediction of illness condition and provide guidance for clinical treatment.
