2019年贵州省228家医疗机构医院感染监测平台数据分析 被引量:6

Data from monitoring platforms of nosocomial infection in 228 healthcare institutions of Guizhou in 2019

摘要 目的对在贵州省医院感染监测数据平台上报的医院感染监测数据进行分析,了解该省不同地区不同等级医疗机构医院感染基本情况及特点。方法医疗机构通过卫生专网登录院感监测平台上报监测数据,对参与监测医院2019年1月1日-12月31日的医院感染相关指标进行收集分析。结果共收集全省228家医疗机构医院感染相关质控数据,医院感染发病率、现患率分别为0.62%、1.14%,医院感染漏报率为4.29%,多药耐药菌医院感染发生率和检出率分别为0.04%、12.96%,手卫生依从率为78.41%,住院患者抗菌药物使用率及使用前病原学送检率分别为48.54%、36.24%,Ⅰ类切口的手术部位感染率、抗菌药物预防使用率分别为0.37%、28.47%,血管内导管相关血流感染发病率、呼吸机相关肺炎发病率、导尿管相关泌尿道感染发病率分别为0.514‰、3.765‰、1.199‰。对比不同地区不同等级、不同床位数医疗机构医院感染相关指标率,其差异均具有统计学意义(P<0.001)。结论本次调查掌握了贵州省不同地区不同等级、不同床位数医疗机构的医院感染特点和差异,为有针对性地指导各地区、各级、各规模医疗机构做好医院感染防控工作提供了科学依据。 OBJECTIVE To analyze the data reported from the monitoring platforms of nosocomial infection in Guizhou province and understand the basic situation and characteristics of nosocomial infection in different levels of medical institutions. METHODS The healthcare institutions reported the monitoring data by logging in the monitoring platforms, and the nosocomial infection-related indexes that were involved in the monitoring from Jan 1, 2019 to Dec 31, 2019 were collected and analyzed. RESULTS The nosocomial infection-related quality control data were collected from 228 medical institutions of the whole province, the incidence rate and prevalence rate of nosocomial infection were 0.62% and 1.14%, respectively, the missing report rate of nosocomial infection was 4.29%. The incidence rate and isolation rate of multidrug-resistant organisms infections were 0.04% and 12.96%, respectively. The hand hygiene compliance was 78.41%. The utilization rate of antibiotics and etiologic submission rate before use of antibiotics of hospitalized patients were 48.54% and 36.24%, respectively. The incidence of surgical site infection and rate of prophylactic use of antibiotics of type Ⅰ incision patients were 0.37% and 28.47%, respectively. The incidence rates of vascular catheter-related bloodstream infection, ventilator-associated pneumonia and catheter-related urinary tract infection were 0.514‰, 3.765‰ and 1.199‰, respectively. There were significant differences in the nosocomial infection-related indexes among the different scales of medical institutions with different numbers of beds in different areas(P<0.001). CONCLUSION The survey facilitates the understanding of the characteristics and differences of nosocomial infection in the different scales of medical institutions with different numbers of beds in different areas of Guizhou province so as to provide scientific basis for targeted prevention and control of nosocomial infection in the medical institutions.
