胃肠道肿瘤患者术后胃肠功能恢复的影响因素分析

Analysis of infl uencing factors of gastrointestinal function recovery in patients with gastrointestinal tumors

目的探讨胃肠道肿瘤患者术后胃肠功能恢复的影响因素。方法选择2018年1月—2019年12月行根治性切除术治疗的胃肠道肿瘤患者164例作为研究对象,根据术后排气时间和排便时间分为胃肠功能恢复正常组(3~7 d)和较差组(>7 d),总结患者的临床资料及治疗记录,分析其影响胃肠功能恢复的相关因素。结果两组患者体质指数、腹腔积液、手术时间、术中出血量、术中补液量、术后开始活动时间比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,体质指数、腹腔积液、手术时间、术中出血量、术中补液量、术后开始活动时间为影响胃肠道肿瘤患者术后胃肠功能的独立因素。结论胃肠道肿瘤患者术后胃肠功能恢复的影响因素较多,肥胖、腹腔积液、手术时间长、术中出血量、补液量多及术后未行胃肠功能锻炼的患者为高危人群,应针对性做好护理干预,以促进术后胃肠功能的恢复。

Objective To explore the infl uencing factors of gastrointestinal function recovery in patients with gastrointestinal tumors.Methods To select 164 patients with gastrointestinal tumors who underwent radical resection from January 2018 to December 2019 as the study objects.According to the postoperative exhaust time and defecation time,they were divided into a normal gastrointestinal function group(3~7 days)and a poor group(>7 days),we summarized the patient's clinical data and treatment records,and analyzed the related factors that affected the recovery of gastrointestinal function.Results There were statistically significant differences in the body mass index,abdominal effusion,operation time,intraoperative blood loss,intraoperative fluid supplementation,and postoperative start time between the two groups of patients(P<0.05).Multivariate Logistic regression analysis showed that the body mass index,abdominal eff usion,operation time,intraoperative blood loss,intraoperative fl uid supplementation,and postoperative start time were the independent factors that aff ected the gastrointestinal function of patients with gastrointestinal tumors after surgery.Conclusion There were many factors infl uencing the recovery of gastrointestinal function in patients with gastrointestinal tumors.The high-risk populations who were obese,effusions in the abdominal cavity,long operation time,intraoperative blood loss,excessive fl uid supplementation,and postoperative gastrointestinal function exercises should be targeted for nursing intervention to promote postoperative gastrointestinal recovery.
