Experience of Professor Yu Guran in Treating Intracranial Tinnitus with Invigorating Kidney for Subsiding Yang Prescription

脑鸣是自觉脑内如虫蛀鸣响的临床常见疾病,其病因病机复杂,西医目前尚无有效的治疗措施。于顾然教授认为脑鸣的病位在脑,脑为髓海,脑的正常功能活动依赖于脑髓这一基本物质,髓海不足,则脑转耳鸣,肾藏精,精能生髓,精髓上充于脑髓,因此脑鸣与肾密切相关,临床上多见本虚标实之证,以肾精亏虚为本,肝阳、湿浊、瘀血为标,病机可涉及肝、脾、肾三脏,其中肾精亏虚是其始动因素,且涉及脑鸣整个病变过程之中,治疗上当以补肾填精为主,兼以平肝潜阳、健脾化湿、活血化瘀等治标之法,临床上多运用补肾潜阳方治疗脑鸣,疗效显著。

Intracranial tinnitus is a common clinical disease with complex etiology and pathogenesis.There is no effective treatment in Western medicine at present.Professor Yu Guran holds that the location of intracranial tinnitus is in the brain.The brain is the medullary sea,and the normal function and activity of the brain depend on the basic substance of the medullary sea.If the medullary sea is insufficient,the brain will turn to tinnitus,and the kidney will store essence,which can produce marrow,and the essence will be filled in the brain medullary.It can be seen that the intracranial tinnitus is closely related to the kidney.Clinically,Professor Yu believes that more of this disease is deficiency in origin and excess in superficiality and kidney essence deficiency as the base,liver yang,dampness turbidity,blood stasis as the superficiality.The pathogenesis involves in liver,spleen,and kidney,deficiency of kidney essence is the initiating factor and involves in the whole pathological process of the disease.The key to treatment is to tonify the kidney and essence,repress hyperactive liver yang,invigorate spleen for eliminating dampness and promote blood circulation to dissipate blood stasis etc of treats in superficiality concurrently.Professor Yu used the invigorating kidney for subsiding yang prescription for syndrome differentiation to treat intracranial tinnitus in clinical practice,which takes significant effects.
