腹腔镜与开腹单侧卵巢良性肿物剔除术对卵巢储备功能的影响比较

Effect of Laparoscopic and Open Unilateral Removal of Benign Ovarian Mass on Ovarian Reserve Function

目的探究腹腔镜与开腹单侧卵巢良性肿物剔除术对卵巢储备功能的影响。方法选取2018年8月至2020年10月在柘城县人民医院接受卵巢良性肿物剔除术的48例患者作为研究对象,以治疗方式分为两组,观察组(腹腔镜术)30例,对照组(开腹术)18例。评估两组治疗效果,收集两组手术相关指标,获取卵巢基质血流参数[阻力指数(RI)、收缩期峰值(PSV)、舒张期末血流速度(EDV)],并检测血清抗苗勒氏管激素(AMH)、基础卵泡刺激素(bFSH)。结果两组患者疗效比较,差异无统计学意义(P>0.05)。观察组肛门排气、下床活动及住院时间均短于对照组(P<0.05)。术后1个月,两组RI、bFSH均高于术前,PSV、EDV及AMH均低于术前,术后6个月,两组RI、bFSH低于术后1个月,PSV、EDV、AMH高于术后1个月(P<0.05)。结论腹腔镜良性肿物剔除术治疗效果及对卵巢储备功能的影响与开腹术相当,且能促进患者快速康复。

Objective To investigate the effect of laparoscopic and open unilateral removal of benign ovarian mass on ovarian reserve function.Methods Forty-eight patients who underwent ovarian benign tumor removal in Zhecheng people’s Hospital from August 2018 to October 2020 were selected as the research subjects.All patients were divided into two groups according to the treatment method,30 in observation group(laparoscopic)and 18 in control group(laparotomy).Efficacy of the two groups was evaluated,surgery-related indicators of the two groups were collected,ovarian stromal blood flow parameters[resistent index(RI),peak systolic velocity(PSV),end-diastolic velocity(EDV)]were obtained,and serum biochemical indicators[anti-Müllerian hormone(AMH),basal follicle stimulating hormone(bFSH)]were detected.Results There was no statistical difference on efficacy between two groups(P>0.05).The anal exhaust time,activity time out of bed and hospitalization time in observation group were lower than those in control group(P<0.05).At 1 month after surgery,RI and bFSH were higher than preoperative,and PSV,EDV,AMH were lower than preoperative.At 6 months after surgery,RI and bFSH were lower than at 1 month after surgery,and PSV,EDV,AMH were higher than at 1 month after surgery(P<0.05).Conclusion The treatment effect of laparoscopic benign mass removal and the effect on ovarian reserve function are comparable to that of laparotomy,and can promote the rapid recovery of patients.
