CT引导下腹腔穿刺引流术治疗急性胰腺炎患者的效果

Efficacy of CT-Guided Abdominal Puncture and Drainage in the Treatment of Acute Pancreatitis

目的分析CT引导下腹腔穿刺引流术治疗急性胰腺炎(AP)患者的效果。方法回顾性选取安阳市第五人民医院2018年4月至2020年10月收治的74例AP患者,根据手术方法分为两组,两组均接受基础治疗。开腹组37例接受常规开腹引流术,CT组37例行CT引导下腹腔穿刺引流术。比较两组C反应蛋白(CRP)、血清淀粉酶恢复正常时间、症状消失时间、住院时间、并发症发生率及术后1、3、5、7 d急性生理与慢性健康评分(APACHEⅡ)。结果与开腹组比较,CT组血清淀粉酶、CRP恢复时间及症状消失时间、住院时间较短,术后3、5、7 d APACHEⅡ评分较低(P<0.05);两组并发症发生率比较,CT组(5.41%)低于开腹组(24.32%)(P<0.05)。结论应用CT引导下腹腔穿刺引流术治疗AP患者效果显著,能迅速减轻临床症状,控制病情,促进机体恢复,缩短病程,且安全性相对较高。

Objective To analyze the efficacy of CT-guided abdominal puncture and drainage in the treatment of acute pancreatitis(AP).Methods A retrospective selection of 74 AP patients in Anyang Fifth People’s Hospital from April 2018 to October 2020 were grouped according to the surgical method.The patients were given basic treatment.Thirty-seven patients in the open group received conventional open drainage,and 37 patients in the CT group underwent CT-guided abdominal puncture and drainage.C-reactive protein(CRP),serum amylase recovery time,symptom disappearance time,hospital stay,complication rate and postoperative(1,3,5,7 d)acute physiology and chronic health scores(APACHE Ⅱ)were compared between the two groups.Results Compared with the laparotomy group,the CT group had shorter serum amylase,CRP recovery time,symptom disappearance time,and shorter hospital stay,and lower APACHE Ⅱscores on 3,5,and 7 days after surgery(P<0.05).The complication rate of CT group(5.41%)was lower than that of open group(24.32%)(P<0.05).Conclusion The application of CT-guided abdominal puncture and drainage in the treatment of AP patients has a significant effect.It can quickly reduce clinical symptoms,control the condition,promote the recovery of the body,shorten the course of the disease,and it is relatively safe.
