摘要
目的探讨高频超声联合血清肿瘤特异性生长因子(TSGH)检测诊断甲状腺癌的临床效果。方法选取2018年5月至2020年5月间上海市宝山区罗店医院收治的60例甲状腺癌患者为甲状腺癌组,另选取同期60例良性甲状腺肿瘤患者为良性肿瘤组,采用彩色多普勒超声诊断仪进行高频超声检查,采用多功能酶标仪运用分光光度法测定吸光度值(OD值),计算血清TSGH水平,分析高频超声联合TSGH检测用于甲状腺癌诊断中的效果。结果60例甲状腺癌组患者中,高频超声显示颈部淋巴结肿大25例;血流阻力指数>0.7有16例,血流呈高阻力型;有沙粒样钙化状出现在甲状腺内9例,钙化光点1~3mm,形态不规则、边界清晰,呈蟹足状或锯齿状;有丰富血流者6例,液性暗区不规则。甲状腺癌组患者血清TSGH水平和TSGH阳性率均高于良性肿瘤组,其中有淋巴结转移患者的血清TSGH水平高于无淋巴结转移患者和良性肿瘤组,阳性率高于无淋巴结转移患者和良性肿瘤组,差异均有统计学意义(均P<0.05)。但无淋巴结转移患者和良性肿瘤组患者血清TSGH水平和阳性率比较,差异均无统计学意义(P>0.05)。结论高频超声联合TSGH检测诊断甲状腺癌临床效果好。
Objective To observe the efficacy of high frequency ultrasound combined with detection of tumor specific growth factor(TSGF)for thyroid cancer.Methods Sixty patients with thyroid cancer who were admitted to Luodian Hospital were selected from May 2018 to May 2020 and were included in a thyroid cancer group.Another 60 patients with benign thyroid tumors were selected and included in a benign tumor group.Color Doppler ultrasound was used to perform high frequency ultrasound examination.Multifunctional enzyme reader and spectrophotometry were used for absorbance also known as optical density(OD).The serum TSGH level was calculated.The efficacy of high frequency ultrasound combined with detection TSGH for thyroid cancer was analyzed.Results Among the 60 patients in the thyroid cancer group,high-frequency ultrasound showed that 25 patients had enlarged lymph node in the neck.In 16 patients,resistance index was above 0.7 and resistance to blood flow was high.Nine patients had sand-like calcification in the thyroid gland with bright calcification specks or dots between 1 and 3 mm,irregular shape,clear boundary and crab foot-or serrate-like shape.Six patients had abundant blood flow and irregular fluid-like dark areas with a proportion of 41.7%,26.7%,15.0%and 10.0%,respectively.Serum TSGH level and positive rate of TSGH were significantly higher in the thyroid cancer group than those in the benign tumor group(all P<0.05).The serum TSGH level and positive rate of fluid-like were significantly higher in patients with lymph node metastasis than those without lymph node metastasis and patients in the benign tumor group(all P<0.05).However,there was no significant difference in serum TSGH level and positive rate of TSGH between patients without lymph node metastasis and patients with benign tumors(P>0.05).Conclusion High frequency ultrasound combined with the detection of TSGH shows good efficacy for the diagnosis of thyroid cancer.
