摘要
目的探讨宫腔镜下宫颈锥切术(TCRC)治疗宫颈高级别鳞状上皮内瘤变(HSIL)合并人乳头瘤病毒感染(HPV)患者的效果及对HPV清除疗效的影响。方法选取2019年8月至2020年10月间榆林市第二医院收治的96例HSIL合并HPV患者为研究对象。依据随机数表法将入选患者分为观察组与对照组,每组48例。对照组行宫颈冷刀锥切术治疗,观察组实施TCRC治疗。观察两组患者的手术效果、HPV感染清除总有效率及术后并发症发生情况。结果与对照组相比,观察组患者的手术时间、住院时间及切口愈合时间显著缩短,术中出血量更少,差异均有统计学意义(均P<0.05)。与对照组的79.2%相比,观察组的HPV清除总有效率为95.8%,差异有统计学意义(P<0.05)。与对照组的25.0%相比,观察组的并发症总发生率为4.2%,差异有统计学意义(P<0.05)。结论TCRC治疗HSIL合并HPV患者疗效切确,HPV能够得到有效清除,术后并发症少,安全性更高。
Objective To explore the effect of transcervical resection of the cervix(TCRC)in the treatment of high-grade cervical squamous intraepithelial lesion(HSIL)complicated with human papillomavirus(HPV)infection and its effect on the efficacy of clearance of HPV.Methods Ninety-six patients with HSIL and HPV admitted to Yulin Second Hospital were selected from August 2019 to October 2020.Using the random number table method,the patients were divided into an observation group and a control group with 48 patients in each group.The observation group underwent TCRC and the control group underwent cold knife conization.The efficacy of surgery,overall efficacy rate of clearance of HPV and postoperative complications were compared between the two groups.Results Compared with the control group,the duration of operation,length of hospital stay and time to incision healing were significantly short and the intraoperative blood loss was minimal in the observation group(all P<0.05).The overall efficacy rate of clearance of HPV was 95.8 in the observation group compared with 79.2%of the control group(P<0.05).The overall incidence of complications was 4.2%in the observation group compared with 25.0%of the control group(P<0.05).Conclusion TCRC is effective in the treatment of HSIL patients with HPV.HPV can be effectively cleared with low postoperative complications and high safety.
