不同模式延续护理对膝关节前交叉韧带重建术后患者家庭康复训练依从性及功能的影响

Effects of different modes of continuous nursing on compliance and function of family rehabilitation training after reconstruction of anterior fork ligament of knee joint

目的探讨2种不同模式延续护理干预对膝关节前交叉韧带(ACL)重建术后患者家庭康复训练依从性及功能的影响。方法选取2019年6月-2020年5月于皖南医学院第一附属医院弋矶山医院关节骨科接受初次单侧ACL重建术的患者88例,采用随机数字表法分为对照组和观察组各44例,对照组采用电话回访模式进行延续护理干预,观察组采用一对一、点对点的个性化微信模式进行延续护理干预。对2组患者出院时、干预后2周、1个月、3个月及6个月家庭康复训练依从性和膝关节功能情况进行比较。结果出院时,2组患者家庭康复训练依从性及膝关节功能比较,差异均无统计学意义(P>0.05);干预后2周,观察组家庭康复训练依从性高于对照组(P<0.05),但膝关节功能比较,差异无统计学意义(P>0.05);干预后1个月、3个月及6个月时,观察组家庭康复训练依从性及膝关节功能均优于对照组,差异有统计学意义(P<0.05)。结论采用一对一、点对点的个性化微信模式对ACL重建术后患者进行延续护理干预优于电话回访模式延续护理干预,能解除患者及家属的思想顾虑,保护患者隐私,提高其家庭康复训练依从性,有利于膝关节功能的恢复,值得临床推广应用。

Objective To investigate the effects of different modes of continuous nursing intervention on the compliance and function of family rehabilitation training after ACL reconstruction.Methods A total of 88 patients discharged from the Department of Orthopedics of our hospital after arthroscopic ACL reconstruction during June 2019 to May 2020 were randomly divided into the telephone visit group(n=44)and the WeChat group(n=44).Patients in the telephone interview group received telephone return visits to continue nursing intervention, and the WeChat group used WeChat video to continue nursing intervention.The two groups were compared regarding the compliance of rehabilitation training and the recovery of knee function in different time periods after intervention.Results There was no statistical significance in the compliance of rehabilitation training and knee function between the two groups at discharge(P>0.05).Compliance of rehabilitation training: the WeChat group was higher than that of the telephone visit group at 2 weeks, 1,3,6 months after intervention,(P<0.05).Comparison of knee function: there was no statistical significance between the two groups at2 weeks after intervention(P>0.05);but the WeChat group was better than that of the electrical visit group at 1, 3, 6 months after intervention(P<0.05).Conclusions WeChat one-to-one personalized continuing care for patients after anterior cruciate ligament reconstruction is better than telephone follow-up continuing care. It can protect patients’ privacy, reduce patients’ concerns, improve patients’ compliance with rehabilitation training and promote the recovery of knee function, which is worthy of promotion and application.
