加温湿化高流量鼻导管吸氧治疗早产儿呼吸暂停的疗效及血气指标的分析 被引量:4

摘要 目的:探讨加温湿化高流量鼻导管吸氧(HHFNC)治疗早产儿呼吸暂停的疗效及对血气指标作用。方法:2019年1月-2020年6月收治呼吸暂停早产儿70例,随机分为两组,各35例。对照组在常规治疗上给予经鼻持续气道正压通气(nCPAP);观察组在常规治疗上给予HHFNC。比较两组患儿临床疗效及并发症发生率。结果:两组临床总有效率比较,差异无统计学意义(P>0.05);观察组并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。治疗后72 h,两组二氧化碳分压(PaCO_(2))较治疗前均降低,动脉血氧分压(PaO_(2))、氧合指数(OI)升高,观察组指标变化幅度大于对照组,组间、不同时间点比较差异有统计学意义(P<0.05)。结论:HHFNC和nCPAP治疗早产儿呼吸暂停的疗效相似,HHFNC治疗同样可有效改善血气指标,HHFNC治疗的安全性更好。 Objective:To investigate the effect of warming and humidifying high-flow nasal cannula for oxygen inhalation(HHFNC)in the treatment of apnea of premature infants and its effect on blood gas index levels.Methods:From January 2019 to June 2020,70 cases of premature infants with apnea were selected,they were randomly divided into the two groups with 35 cases in each group.The control group was given nasal continuous positive airway pressure(nCPAP)treatment.The observation group was given HHFNC treatment.We compared the clinical efficacy and the incidence of complications between the two groups of children.Results:There was no statistically significant difference in the clinical total effective rate between the two groups(P>0.05).The incidence of complications in the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05);after the treatment 72 hours,the partial pressure of carbon dioxide(PaCO_(2))in the two groups was lower than that before the treatment,and the partial pressure of arterial oxygen(PaO_(2)),oxygenation index(OI)were rised,the amplitude in the observation group was larger than that of the control group,and the difference was statistically significant between the interblock and at different time points(P<0.05).Conclusion:HHFNC and nCPAP have similar effects in the treatment of apnea in premature infants,HHFNC can also improve blood gas index,but HHFNC has better safety.
