超早期小骨窗微创脑出血清除术治疗高血压脑出血的疗效分析

Efficacy analysis of ultra-early small bone window minimally invasive cerebral hemorrhage clearance in the treatment of hypertensive intracerebral hemorrhage

目的:探讨超早期小骨窗微创脑出血清除术应用于高血压脑出血治疗中的效果。方法:2018年1月-2020年1月收治高血压脑出血(HCH)患者100例,随机分为两组,各50例。对照组予以早期(发病后6~72 h)小骨窗微创术;研究组予以超早期(6 h内)小骨窗微创手术。比较两组临床效果。结果:研究组手术时间、术中失血量、拔管时间、术后住院时间均低于对照组,差异有统计学意义(P<0.05)。研究组术后1个月格拉斯哥昏迷指数(GCS)评分高于对照组,神经功能缺损量表(NFDS)评分低于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:超早期手术可有效缩减手术时间、减少术中出血,有利于进一步提升患者临床表现、更好地保护脑神经功能,且并发症更少。

Objective:To investigate the effect of ultra-early small bone window minimally invasive cerebral hemorrhage clearance in the treatment of hypertensive cerebral hemorrhage.Methods:From January 2018 to January 2020,100 patients with hypertensive cerebral hemorrhage were selected,they were randomly divided into the two groups with 50 cases in each group.The control group received early(6 to 72 hours after onset)small bone window minimally invasive surgery.The study group received ultra-early(within 6 hours)small bone window minimally invasive surgery.We compared the clinical effects of the two groups.Results:The operation time,intraoperative blood loss,extubation time,and postoperative hospital stay in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The GCS score of the study group was higher than that of the control group one month after operation,and the NFDS score was lower than that of the control group,the differences were statistically significant(P<0.05).The incidence of complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Ultra-early surgery can effectively shorten the operation time and reduce intraoperative bleeding,which is beneficial to further improve the clinical performance of the patients,better protect the cranial nerve function,and have fewer complications.
