脐部双通道两孔腹腔镜阑尾切除术

Double-channel two-hole laparoscopic appendectomy of the umbilical cord

目的探讨脐部双通道两孔腹腔阑尾切除术的可行性、手术技巧及优势。方法回顾分析2019年11月至2020年2月30例在本院行脐部双通道两孔腹腔阑尾切除术患者的临床资料、手术过程、术后恢复情况。结果本组患者均通过在脐窝下缘建立双通道,耻骨上区建立5mm通道,通过常规器械,行腔镜打结,电凝切断阑尾及系膜等方法顺利完成两孔腹腔镜阑尾切除术,手术时间(30.1±7.2)分钟,术后下床时间(10.3±4.0)h,排气时间(1.6±0.76)d,患者术后无肠梗阻、腹腔积液、粪漏、出血等并发症,术后(2.8±1.0)d出院。结论脐部双通道腹腔阑尾切除术安全可行,具有切口小、隐蔽,操作方便无需特殊器械等优点,值得推广应用。

Objective To explore the feasibility,surgical skills and advantages of double-channel abdominal appendectomy in the umbilical cord.Methods The clinical data,surgical procedure and postoperative recovery of 30 double-channel bilateral abdominal appendectomy from November 2019 to February 2020 were analyzed.Results Two-hole laparoscopic appendectomy by establishing double channel at the lower edge of the umbilical socket,upper pubic area,through conventional instruments,ligation,and appendix and membrane,30.1±7.2 minutes,10.3±4.0 hours,exhaust time 1.6±0.76 days.Patients had no intestinal obstruction,abdominal effusion,fecal leakage,bleeding and other complications and discharged for 2.8±1.0 days after surgery.Conclusion Double-channel abdominal appendectomy of the umbilical cord is safe and feasible,with the advantages of small incision,hidden,and convenient operation without special devices,which is worth popularization and application.
