天津市健康检查人群中代谢性疾病与肾功能异常的相关分析

Correlation analysis between metabolic diseases and abnormal renal function among health check crowd in Tianjin

目的探讨天津市健康检查人群中代谢性疾病(高血压、糖尿病、肥胖和高脂血症)与肾功能异常的相关性。方法采用整群抽样的方法招募2018年9月—2019年12月在在天津市人民医院和天津市第一中心医院健康管理中心进行健康检查的成年人作为调查对象,问卷收集其人口学特征、既往病史、生活行为习惯,测量血压、身高和体质量,并采集血液检测空腹血糖、三酰甘油、总胆固醇、低密度脂蛋白胆固醇和血肌酐,并对数据进行分析。结果共调查18 774人,其中16 795人纳入分析。研究人群中高血压、糖尿病、肥胖和高脂血症的患病率分别为30.8%、9.4%、17.5%和24.8%,肾功能异常患病率为15.4%。高血压、糖尿病和高脂血症患者发生肾功能异常的风险分别是非患者的1.63倍(95%CI:1.48~1.78)、1.24倍(95%CI:1.09~1.42)和1.47倍(95%CI:1.33~1.62),而肥胖人群肾功能异常患病率与非肥胖人群比较,差异无统计学意义(P>0.05)。结论高血压、糖尿病和高脂血症与肾功能异常相关,尚未发现肥胖与肾功能异常相关。

Objective To explore the correlation between metabolic diseases(hypertension,diabetes,obesity and hyperlipidemia)and abnormal renal function among health check crowd in Tianjin. Methods The cluster sampling was use d to recruit adults who received the health examination in the Tianjin People’s Hospital and the health management center of Tianjin First Central Hospital from September 2018 to December 2019 as the survey object. The demographic characteristics,previous medical history,living habits,blood pressure,height and body mass were measured by the questionnaire,the blood samples were collected to detect fasting blood glucose,triglyceride,total cholesterol,low density lipoprotein cholesterol and blood creatinine. And the data were analyzed. Results A total of 18 774 people were investigated and 16 795 were included. The prevalence of hypertension,diabetes,obesity and hyperlipidemia in survey objects was 30.8%,9.4%,17.5% and 24.8%,respectively. The prevalence of abnormal renal function was 15.4%. Multivariate logistic regression showed the risk of abnormal renal function among patients with hypertension,diabetes and hyperlipidemia was respectively 1.63(95%CI:1.48-1.78),1.24(95%CI:1.09-1.42) and 1.47(95%CI:1.33-1.62) times that of people without the diseases. There was no statistically significant difference in the prevalence of renal dysfunction between obese and non obese population(P>0.05). Conclusion Hypertension,diabetes and hyperlipidemia are associated with abnormal renal function,however,no association is found between obesity and renal dysfunction.
