全科团队式管理在轻中度阿尔茨海默病患者中的应用

Application of general practice team management in patients with mild to moderate Alzheimer′s disease

目的探讨全科团队式管理模式在轻中度阿尔茨海默病(AD)患者中的应用效果。方法选取2016年1月至2019年12月上饶市人民医院和上饶市信州区北门街道社区卫生服务中心随诊的轻中度AD患者共96例作为研究对象,按照随机数字表法分为试验组和对照组,各48例。试验组进行全科团队式管理,对照组进行常规自我管理。分析两组患者盐酸多奈哌齐和盐酸美金刚的服药率、简易智能状态检查量表(MMSE)评分、日常生活活动能力量表(ADL)评分、因病入院率、家庭照护者Zarit负担问卷(ZBI)评分。结果随访1年后,试验组服药率高于对照组,差异有统计学意义(P<0.05);干预后,试验组MMSE评分、ADL评分高于对照组,因病入院率及ZBI评分低于对照组,差异均有统计学意义(P<0.05)。结论对轻中度AD患者进行全科团队式管理可以提高治疗依从性,病情控制更佳,同时能减轻家庭照护者负担,效果较好。

Objective To explore the effect of general practice team management in patients with mild to moderate Alzheimer′s disease(AD).Methods A total of 96 patients with mild to moderate AD who were followed up by Shangrao People′s Hospital and Beimen Street Community Health Service Center,Xinzhou District,Shangrao City from January 2016 to December 2019 were selected as the research objects,and were divided into experimental group and the control group according to the random number table method,each with 48 cases.The experimental group was managed by a general practice team,and the control group was managed by routine self-management.Analyze the dopezil hydrochloride and memantine hydrochloride medication rates,mini-mental state examination(MMSE)scores,activities of daily living(ADL)scores,hospital admission rates,and family caregivers′Zarit burden questionnaire(ZBI)between the two groups of patients score.Results After 1 year of follow-up,the rate of medication in the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).After intervention,the MMSE score and ADL score of the experimental group were higher than those of the control group,and the hospital admission rate and ZBI score were lower than the control group,the differences were statistically significant(P<0.05).Conclusion General practice team management for patients with mild to moderate AD can improve treatment compliance,better disease control,and reduce the burden on family caregivers with better results.
