摘要
目的探讨磁共振成像(MRI)检查在评估膝关节退行性骨关节炎患者关节损伤程度中的价值。方法回顾性分析2019年7月至2020年7月期间赣州市人民医院收治的130例膝关节退行性骨关节炎患者的临床资料,所选患者行MRI检查,以关节镜检查结果作为金标准,观察MRI检查的诊断价值,比较两种检查方式对关节炎症状检出率以及对关节软骨损伤分级情况。结果MRI检查对关节软骨损伤诊断的灵敏度为93.20%(96/103),特异度为70.37%(19/27),准确度为88.46%(115/130)。两种检查方式的关节间隙狭窄、关节腔内游离体、骨质增生、滑膜增厚改变、韧带退变、半月板退行性损伤以及关节积液检出率比较,差异无统计学意义(P>0.05)。两种检查方式对软骨损伤分级情况比较,差异无统计学意义(P>0.05)。结论MRI检查评估膝关节退行性骨关节炎关节软骨损伤程度与关节镜相近,且MRI的灵敏度与特异度也较高。
Objective To explore the evaluation value of magnetic resonance imaging(MRI)in patients with knee degenerative osteoarthritis for the degree of joint damage.Methods Retrospective analysis of the clinical data of 130 patients with knee degenerative osteoarthritis admitted to Ganzhou People′s Hospital from July 2019 to July 2020.All of the patients were examined by MRI,and used the arthroscopic results as the gold standard,observed the diagnostic value of the MRI,and the detection rate of arthritis symptoms and the classification of articular cartilage injury were compared.Results The sensitivity of MRI to the diagnosis of articular cartilage injury was 93.20%(96/103),specificity of 70.37%(19/27),and accuracy of 88.46%(115/130).There were no significant differences in the detection rate of joint gap stenosis,joint cavity free body,osteogenic hyperplasia,altered synovial thickening,ligament degeneration,meniscus degenerative injury,and joint effusion between the two examination methods(P>0.05).There was no significant difference in graded of the cartilage injury between the two examination methods(P>0.05).Conclusion MRI examination assessed knee degenerative osteoarthritis articular cartilage damage was similar to arthroscopy,and the sensitivity and specificity of MRI were also high.
