葛根芩连汤联合替硝唑治疗成人慢性牙周炎疗效及对患者牙周菌斑分布、炎症水平的影响

Curative Effect of Gegen Qinlian Decoction Combined with Tinidazole for Adults with Chronic Periodontitis and Its Influences on the Distribution of Periodontal Plaque and Inflammation Level

目的:观察葛根芩连汤联合替硝唑治疗成人慢性牙周炎的疗效以及对患者牙周菌斑分布、炎症水平的影响。方法:将108例慢性牙周炎患者随机(简单随机分组法)分为联合组和对照组,各54例,分别给予葛根芩连汤+替硝唑和单一替硝唑治疗,连续治疗4w,比较两组疗效、中医证候积分、牙周状况及炎症水平。结果:联合组临床总有效率显著高于对照组(92.59%vs 75.93%,P<0.05);治疗后,两组牙龈肿痛、口臭、口燥咽干、齿衄、便秘方面的中医证候积分均较同组治疗前明显降低,且联合组治疗后各方面中医证候积分显著低于对照组(P<0.05);治疗后,两组菌斑指数(PLI)、牙周袋探诊深度(PD)、龈沟出血指数(SBI)、牙齿松动度(TM)较同组治疗前明显降低,且联合组治疗后各指标显著低于对照组(P<0.05);治疗后,两组白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平较同组治疗前明显降低,且联合组治疗后IL-6、IL-1β、TNF-α水平显著低于对照组,有统计学差异(P<0.05)。结论:葛根芩连汤联合替硝唑治疗成人慢性牙周炎的疗效显著,不仅能缓解患者相关疾病表现症状,还能改善牙周状况,减少牙周菌斑分布,降低炎性因子水平,促进疾病康复。

Objective:To explore the curative effect of Gegen Qinlian Decoction combined with Tinidazole for adults with chronic periodontitis and its influences on the distribution of periodontal plaque and inflammation level.Methods:A total of 108 patients with chronic periodontitis were randomly(simple random grouping method)divided into a combination group(Gegen Qinlian Decoction combined with Tinidazole)and a controlled group(Tinidazole),54 cases in each group.They were continuously treated for 4 weeks.The curative effect,scores of TCM syndromes,periodontal condition and inflammation level were compared between the two groups.Results:The total clinical response rate in the combination group was significantly higher than that in the controlled group(92.59%vs 75.93%,P<0.05).After treatment,scores of TCM syndromes such as gum swelling,halitosis,dry mouth,gingival hemorrhage and constipation in both groups were significantly decreased,which were significantly lower in the combination group than in the controlled group(P<0.05).After treatment,plaque index(PLI),probing depth(PD),sulcus bleeding index(SBI)and tooth movement(TM)in both groups were significantly decreased,which were significantly lower in the combination group than in the controlled group(P<0.05).After treatment,levels of interleukin-6(IL-6),interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)in both groups were significantly decreased,which were significantly lower in the combination group than in the controlled group(P<0.05).Conclusion:Curative effect of Gegen Qinlian Decoction combined with Tinidazole is significant on adults with chronic periodontitis.It can not only alleviate the symptoms of related diseases,but also improve periodontal condition,reduce the distribution of periodontal plaque,reduce levels of inflammatory factors,and promote their recovery.
