新疆不同社会经济特征居民健康相关生命质量研究

Health-related quality of life of population groups with different socioeconomic characteristics in Xinjiang Uygur Autonomous Region

目的了解不同社会经济特征的新疆居民健康相关生命质量情况及公平性,为有针对性提出改善各群体居民健康相关生命质量,提高健康公平性提供参考建议。方法基于EQ-5D健康量表进行新疆居民健康相关生命质量的调查,使用中国时间均衡法(TTO)计算健康效用值,采用伪基尼系数进行各社会经济分组新疆居民的健康相关生命质量公平性评价。结果新疆居民五个健康维度中中度和极度困难比例最多的是焦虑或抑郁(28.1%)、疼痛或不适(17.06%)。行动能力维度上,各社会经济特征分组之间的差异性无统计学意义;自我照顾能力维度上年龄分组,日常活动能力维度上年龄、户籍、受教育程度、收入水平分组,疼痛或不适维度上户籍、地区、受教育程度、收入水平分组,以及焦虑或抑郁维度上户籍、地区和收入水平分组的中度和极度困难比例差异性具有统计学意义。伪基尼系数显示新疆居民健康效用的公平性从高到低依次是:性别(0.0019)、省会和南北疆分区(0.0032)、户籍(0.0045)、收入水平(0.0105)、受教育程度(0.0118)、年龄(0.0193)。结论要关注新疆居民全方位全生命周期的健康,应对焦虑年轻化问题,发挥高等教育对健康意识和行为的正向作用,扩大新疆中等收入群体,切实提高居民健康水平和公平性,以健康新疆惠及乡村振兴。

Objective To clarify the impact of different socio-economic characteristics on the health-related quality of life and health equity of Xinjiang residents,and to provide reference for the improvement in health-related quality of life and health equity of residents in various groups.Methods The EQ-5 D health scale was used in the questionnaire survey.The health utility value was calculated using the Chinese Time Trade-Off(TTO).The equity of health-related quality of life of Xinjiang residents was evaluated using the pseudo-Gini coefficient.Results Among the five health dimensions,Xinjiang residents with some problems and extreme problems were mostly found in the dimensions of pain/discomfort(17.06%),and anxiety/depression(28.1%).In the dimension of mobility,the difference among the groups with different socio-economic characteristics was not statistically significant.There were statistically significant differences in the rate of some problems and extreme problems in self-care dimension among residents grouped by age,in usual activities dimension among residents grouped by age,registered residence,education and income,in pain/discomfort dimension among residents grouped by registered residence,area,education and income,and in anxiety/depression dimension among residents grouped by registered residence,area and income.The pseudo-Gini coefficient showed that the equity of the health utility of Xinjiang residents ranking from high to low was:gender(0.0019),provincial capital and northern and southern Xinjiang(0.0032),household registration(0.0045),income level(0.0105),education level(0.0118),and age(0.0193).Conclusions It is necessary to attach importance to the health of Xinjiang residents in all aspects and the whole life cycle.The problem of health anxiety and rejuvenation needs to be alleviated.The positive impact of higher education on health awareness and behavior needs to play a part in tackling anxiety in the youth.And it is also necessary to expand population groups with moderate income in Xinjiang,so as to effectively improve the health level and equity of residents,and to promote rural revitalization with healthy Xinjiang.
