泮托拉唑四联疗法与酪酸梭菌活菌联用对幽门螺杆菌阳性胃溃疡患者的疗效及对肿瘤坏死因子-α、白细胞介素-6、白细胞介素-8水平的影响

Efficacy of Pantoprazole Quadruple Therapy Combined with Live Clostridium Caseate on Helicobacter Pylori Positive Gastric Ulcer Patients and Effects on TNF-α, IL-6 and IL-8 Levels

目的探讨泮托拉唑四联疗法与酪酸梭菌活菌联用对幽门螺杆菌(Hp)阳性胃溃疡患者的疗效及对肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平的影响。方法选取2018年2月至2020年2月朝阳市中心医院收治的Hp阳性胃溃疡患者83例作为研究对象,按照随机数字表法分为联合组41例和四联疗法组42例。四联疗法组予以泮托拉唑四联疗法治疗,联合组则于四联疗法组的基础上增用酪酸梭菌活菌治疗。比较两组临床疗效,治疗前后血清胃蛋白酶原(PG)、三叶因子1(TFF1)水平,治疗前后血清TNF-α、IL-6、IL-8水平,组织学再生黏膜成熟度。结果联合组治疗有效率高于四联疗法组(P<0.05)。治疗后联合组血清PG、TFF1水平明显低于四联疗法组(P<0.05)。治疗后联合组血清TNF-α、IL-6、IL-8水平低于四联疗法组(P<0.05)。联合组组织学再生黏膜成熟度高于四联疗法组(P<0.05)。结论泮托拉唑四联疗法与酪酸梭菌活菌联用对Hp阳性胃溃疡患者疗效显著,有利于促进炎症反应的减轻,提高组织学再生黏膜成熟度。

Objective To explore the efficacy of pantoprazole quadruple therapy combined with live clostridium tyrosine on helicobacter pylori(Hp) positive gastric ulcer patients and the effects on the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6) and interleukin-8(IL-8).Methods A total of 83 patients with HP positive gastric ulcer admitted to Chaoyang Central Hospital from February 2018 to February 2020 were selected as study objects.According to the random number table method,41 patients were in the combination group and 42 patients were in the quadruple therapy group.Pantoprazole tetrad was given in quadruple therapy group,and clostridium caseinate live bacteria was added in the combination group on the basis of the quadruple therapy group.The clinical efficacy,serum levels of pepsinogen(PG) and tissue three-leaf factor 1(TFF1) before and after treatment,serum levels of TNF-α,IL-6 and IL-8 before and after treatment,and the maturity of histological regenerated mucosa of the 2 groups were compared.Results The overall treatment effective rate of the combination group was higher than quadruple therapy group(P<0.05).After treatment,the serum levels of PG and TFF1 in combination group were significantly lower than quadruple therapy group(P<0.05).After treatment,the serum levels of TNF-α,IL-6 and IL-8 in combination group were all lower than quadruple therapy group(P<0.05).The maturity of histological regenerated mucosa in combination group was higher than quadruple therapy group(P<0.05).Conclusion Pantoprazole quadruple therapy combined with live clostridium caseate has a significant effect on patients with Hp positive gastric ulcer,which is conducive to promoting the reduction of inflammatory response and improving the maturity of histological regenerated mucosa.
