多巴酚丁胺联合托伐普坦治疗42例难治性心力衰竭的效果分析

Effect of dobutamine combined with tolvaptan in 42 patients with refractory heart failure

目的:分析多巴酚丁胺联合托伐普坦治疗难治性心力衰竭效果。方法:将84例难治性心力衰竭患者根据随机抽样法分成观察组(n=42)和对照组(n=42),对照组给予托伐普坦治疗,观察组给予多巴酚丁胺联合托伐普坦治疗。对比两组治疗前、后左心室收缩末内径(LVESD)、左心室舒张末内径(LVEDD)、左心室射血分数(LVEF)等心功能指标;N末端脑钠肽前体(NT-proBNP)、高敏C反应蛋白(hs-CRP)血清水平;心悸、恶心、头痛、胸痛等不良反应发生率。结果:治疗后,观察组LVESD、LVEDD、NT-proBNP、hs-CRP血清水平低于对照组,LVEF高于对照组(均P <0.05);不良反应低于对照组,但差异无统计学意义(P> 0.05)。结论:多巴酚丁胺联合托伐普坦,可改善RHF患者心功能,在一定程度上减少不良反应。

Objective:To analyze the effect of dobutamine combined with tolvaptan in the treatment of refractory heart failure.Methods:Eighty-four patients with refractory heart failure were randomly divided into an observation group(n=42,treated with dobutamine combined with tolvaptan) and a control group(n=42,treated with tolvaptan).The Heart function indexes such as left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD) and left ventricular ejection fraction(LVEF),the levels of N-terminal pro-brain natriuretic peptide(NT-proBNP) and high-sensitivity C-reactive protein(hs-CRP) and the incidence of adverse reactions such as palpitations,nausea,headache,and chest pain were compared before and after treatment.Results:After treatment,the serum levels of LVESD,LVEDD,NT-proBNP and hs-CRP were lower in the observation group than the control group(all P<0.05).The incidence of adverse reactions in the two groups was lower,but the difference was not statistically significant(P>0.05).Conclusion:Dobutamine combined with tolvaptan can improve cardiac function and reduce the incidence of adverse reactions to some extent.
