肺磨玻璃样结节的CT表现及良、恶性的相关性分析

CT Features of the Pulmonary Ground Glass-Like Nodules and Their Correlation with Benign and Malignant

目的分析肺磨玻璃样结节的CT表现及良、恶性的相关性。方法回顾性分析2017年7月至2019年4月在本院收治的98例肺磨玻璃样结节患者,所有患者都进行MSCT检查。对患者所得MSCT图像进行分析,分析良、恶性结节患者的MSCT表现与其相关性。结果良、恶性结节病灶多均多为规则形态,分别占71.42%,66.66%,良性结节大小为≤1cm占80.35%,恶性结节为66.66%,性结节无胸膜"凹陷征"和血管"集束征"出现,恶性病变胸膜"凹陷征"和血管"集束征"各出现2例4.76%,P>0.05;良性结节73.21%为单纯性磨玻璃结节,边缘与光滑为主(占75.00%),有1例患者出现"毛刺征"(1.78%),恶性结节患者以混合性磨玻璃结节为主(占71.42%),边缘表现为不光滑(47.61%),47.61%患者出现"毛刺征"(P<0.05)。通过多因素Logistic回归分析,肺部膜玻璃结节边缘情况、密度情况、是否有"毛刺征"与其良、恶性病变密切相关(P<0.05),结论肺磨玻璃样结节的CT表现与其良、恶性存在相关性,仔细辨别肺部磨玻璃结节是否有"毛刺征"、血管"集束征"、病灶边缘情况、内部密度等MSCT征象,对其良、恶性鉴别有重大参考价值。

Objective To analyze the CT features of the pulmonary ground glass-like nodules and their correlation with benign and malignant.Methods The data of 98 patients with pulmonary ground glass-like nodules admitted to our hospital from July 2017 to April 2019 were retrospectively analyzed.All patients underwent MsCT.The patient's MsCT images were analyzed.The MsCT features in patients with benign and malignant nodules were analyzed and correlation between MsCT features and benign and malignant nodules was analyzed.Results The lesions of benign and malignant nodules were mostly regular,accounting for 71.42%and 66.66%,respectively.The size of benign nodules was≤1cm,accounting for 80.35%,and malignant nodules accounted for 66.66%.There were no pleural indentation signs and vascular bundle signs in malignant nodules.2 cases showed malignant lesions(4.76%)(P>0.05).73.21%benign nodules were simple ground glass nodules,edge was mainly smooth(75.00%),1 cases showed a burr sign(1.78%).Patients with malignant nodules showed mainly mixed ground glass nodules(71.42%),margins were not smooth(47.61%),and 47.61%patients showed burr signs(P<0.05).Multivariate logistic regression analysis showed that the edge,density,and burr sign of the pulmonary ground glass-like nodules were closely related to benign and malignant lesions(P<0.05).Conclusion There is a correlation between the CT features of the pulmonary ground glass-like nodules and the benign or malignant.To carefully examine MsCT signs such as margins of lesion,internal density,and whether there are burr signs,vascular bundle signs,which has great reference value for the identification of benign and malignant tumor.
