CT与MRI在主动脉夹层诊断中的临床应用价值

Clinical Application Value of CT and MRI in the Diagnosis of Aortic Dissection

目的观察CT与MRI在主动脉夹层(AD)诊断中的临床应用价值。方法对62例AD患者的临床资料进行回顾性分析,根据术前检查方式分为CT组(n=32)和MRI组(n=30)。比较CT与MRI在AD诊断分型以及检出真假腔、撕脱内膜片、内膜破口等的应用价值。结果CT与MRI对AD患者DeBakey分型、真假腔、撕脱内膜片、内膜破口诊断与手术结果符合率均较高,差异无统计学意义(P>0.05)。CT组对主动脉分支受累、主动脉钙化检出率大于MRI组,对附壁血栓检出率小于MRI组(P<0.05)。结论CT与MRI在AD分型、真假腔、撕脱内膜片、内膜破口诊断中均具有良好价值,CT在主动脉分支受累、主动脉钙化检出方面优于MRI,MRI在附壁血栓检出方面优于CT,两种方法各有优劣,临床应根据患者情况适当选择。

Objective To observe the clinical application value of CT and MRI in the diagnosis of aortic dissection(AD).Methods The clinical data of 62 patients with AD were retrospectively analyzed.They were divided into CT group(n=32)and MRI group(n=30)according to the preoperative examination methods.The application value of CT and MRI was compared in the diagnosis of AD classification and in the detection of true and false cavity,avulsion endometrium and endometrial rupture.Results The coincidence rates of CT and MRI in the diagnosis of DeBakey classification,true and false cavity,avulsion endometrium and endometrial rupture of AD patients were higher with surgical results(P>0.05).The detection rates of aortic branch involvement and aortic calcification in CT group were higher than those in MRI group,and the detection rate of lateral thrombus was lower than that in MRI group(P<0.05).Conclusion CT and MRI have good value in the diagnosis of AD classification,true and false cavity,avulsion endometrium and endometrial rupture.CT is superior to MRI in the detection of aortic branch involvement and aortic calcification.MRI is superior to CT in the detection of lateral thrombosis,and the two methods have their own advantages and disadvantages.The methods should be appropriately selected according to the patients’condition in clinical practice.
