血清IL-10、IL-22水平对老年重症支气管哮喘患者气道重构程度预测价值 被引量:13

Predictive value of serum IL-10 and IL-22 levels on airway remodeling in elderly patients with severe bronchial asthma

摘要 目的分析血清白介素-10(IL-10)、IL-22与老年重症支气管哮喘(BA)患者气道重构程度的关系。方法选取2019年1月—2020年1月新疆医科大学第八附属医院重症医学科治疗晚发性非变应性哮喘患者69例作为中度气道重构组(中度组),重度伴气流受限性哮喘患者69例作为重度气道重构组(重度组)。收集2组患者临床资料,检测并分析血清IL-10、IL-22表达与患者气道重构程度的关系及其对气道重构程度的预测价值。结果重度组血清IL-10、IL-22水平均低于中度组(U=5.855、6.093,P均<0.01);WBC、CRP、pH、SaO_(2)、PaO_(2)、PaCO_(2)、FEV1、用力肺活量(FVC)、最大呼气流速(PEF)比较,差异均无统计学意义(P>0.05);老年重症BA患者血清IL-10、IL-22水平与气道重构程度呈负相关(r=-0.437、-0.455,P<0.01);血清IL-10、IL-22水平单独及二者联合预测老年重症BA患者重度气道重构风险的AUC为0.807、0.820、0.879,以二者联合预测价值最佳。结论老年重症BA患者血清IL-10、IL-22水平与气道重构程度有关,可能是老年重症BA患者重度气道重构的危险因素,对患者气道重构程度有一定预测价值。 Objective To analyze the relationship between serum interleukin 10(IL-10),IL-22 and airway remodeling in elderly patients with severe bronchial asthma(BA).Methods From January 2019 to January 2020,69 patients with late-onset non allergic asthma treated by the Department of Critical Medicine of the Eighth Affiliated Hospital of Xinjiang Medical University were selected as the moderate airway remodeling group(moderate group),and 69 patients with severe asthma with airflow restriction were selected as the severe airway remodeling group(severe group).The clinical data of the two groups were collected,and the relationship between the expression of serum IL-10 and IL-22 and the degree of airway remodeling was detected and analyzed.Results The levels of serum IL-10 and IL-22 in severe group were lower than those in moderate group(U=5.855,6.093,P<0.01).There was no significant difference in WBC,CRP,pH,SaO_(2),PaO_(2),PaCO_(2),FEV-1,forced vital capacity(FVC)and maximum expiratory flow rate(PEF)(P>0.05).The levels of serum IL-10 and IL-22 in elderly patients with severe Ba were negatively correlated with the degree of airway remodeling(r=-0.437,-0.455,P<0.01).The AUCs of serum IL-10 and IL-22 levels alone and in combination to predict the risk of severe airway remodeling in elderly patients with severe Ba were 0.807,0.820 and 0.879.Conclusion The levels of serum IL-10 and IL-22 in elderly patients with severe Ba are related to the degree of airway remodeling,which may be a risk factor for severe airway remodeling in elderly patients with severe BA.
